YOUR CONTRIBUTION TO OUR SUCCESS!:
Provide support to the Quality Care Management nursing team and provide outreach  to patients, remove barriers and provide access so that patients are able to successfully complete preventative health measures regardless of payor source, follow up on transition of care and/or chronic care management appointments. The retention of patients is a priority of for the QCC, the ability to connect with patients, provide compassionate care and empowerment is essential. 

 

The QCC contributes by addressing preventive health measure outcomes of all payor source through improving and addressing HEDIS and HIV/AIDS Bureau measures, gaps in care, retention, barriers to care. With radical collaboration with AHF departments and entities, with a solutions-oriented focus on telephonic interaction with patients
