POSITION HIGHLIGHTS: The purpose of this position is to help achieve the vision of Advocate Hospice Services by performing the duties of this position in a cost effective, results oriented and customer focused manner.
This position is a critical component of the interdisciplinary team and as such meeting individual performance expectations and contributions to team performance goals is required.
The Hospice social worker is responsible for the assessment of psychosocial needs of patients and families as well as development of a social work plan of care reflecting problems, interventions and outcome goals WHAT YOU WILL DO: Hospice social worker provides initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services, and community education, outreach and referral.
Formulates and implements an effective care plan to met the patient/family needs Coordinates care and treatment plan with the interdisciplinary group WHAT YOU WILL NEED: Masters degree in social work from an accredited school of social work Minimum of one year experience in a medical clinical setting and/or hospice/palliative care.
Social Work Licensure in state of Illinois (LSW) Experience working with extended families during times of excessive stress, pending deaths and during the process of active dying of loved ones.
