Substance Abuse Case Manager – C3 – Skid Row (Outreach)

Schedule: Monday-Friday (7:30am-4:00pm)

SUMMARY

With the support of and in partnership with the C3 team, the Substance Abuse Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in improving their safety and/or their recovery from substances. The C3 program serves individuals living on the streets of Skid Row in Downtown Los Angeles.

This is part of a LA County-approved strategy to combat homelessness known as E6. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and permanent housing and supportive services. The Substance Use Case Manager provides information and support for program members, and assists members in gaining admission to inpatient or more intensive substance abuse treatment settings as needed. The Substance Use Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations. The Substance Abuse Case Manager will provide intensive case management services to a caseload of highly vulnerable individuals living on the streets of Skid Row.

ESSENTIAL DUTIES & RESPONSIBILITIES

  1. Utilize harm reduction, social, environmental, and motivational, strategies to support clients, in collaboration with licensed clinicians, other counselors, case managers, and support staff.
  2. Encourage and promote an environment that is strength-based to assist clients in meeting their individual goals.
  3. Street outreach to identified homeless persons working with the team to engage in services and accept treatment from the program staff assist with the enrollment process as needed.
  4. Conduct and document individual counseling sessions with clients at regular intervals determined by the individual’s needs.
  5. Coach clients individually on problems with the transition from streets to home, supportive independent living skills, substance use, and accessing needed recourses in the community.
  6. Provide referrals to allied critical healthy living services for clients on caseload. Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.), and collaborate on care.
  7. Document and account for time spent on client contacts and activities in a client record in accordance with agency and funder standards.
  8. Attend and actively participate in team meetings and program-wide meetings daily.
  9. Maintain current, thorough knowledge of ongoing developments and new techniques in the fields of substance abuse, mental health, and behavioral health community resources and utilize these to provide comprehensive, wrap-around services to clients.

QUALIFICATIONS

  1. Certification in substance abuse counseling. (CAADE and/or CAADAC preferred).
  2. Bachelor’s degree in related field preferred.
  3. Minimum two years experience as a substance abuse counselor and/or case manager, in a licensed and/or accredited substance abuse treatment facility (experience in a dual-diagnosis setting preferred).
  4. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
  5. Computer literacy, including knowledge of Microsoft Word, Outlook and Excel
  6. Detail-oriented with excellent time management, organizational, written, verbal, and computer skills
  7. Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
  8. Able to obtain and maintain CPR/1st Aid certification

Preferred Qualifications

  1. Knowledge of the principles and practices of harm reduction, culture, terminology, and literature
  2. Working knowledge of criminal justice system
  3. Bilingual in English/Spanish a plus
  4. Minimum 30 WPM typing speed

Environment and Responsibilities

Work Environment

  • Combination of field and office environment
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist and sit throughout the day
