*** Join Our Team of Highly Talented and Committed Clinical Professionals ***Position :Registered Nurse in Home HealthTYPE: Full-timeLOCATION: Contra Costa Counties (Concord / Martinez areas)SIGN-ON BONUS:$$$$ 1,000 $$$$SIGN-ON BONUS:Company CarPosition Summary:The Neogen Care Hospice RN providesdirect patient care under the supervision of a patients physician Responsibilities include following Start of care plans, providing treatments, Discharges, Wound Care, Medication administrationand working collaboratively with all of the members of the team to help meet positive patient care outcomesEssential Functions:Case management & follow-up skilled nursing visits for hospicepatients.Consult & develop treatment plansbased on physician’s orders and initial patient assessment.Report patient care progress to physician and Clinical staff regularlyDischarge patients after consultation with the physician and Clinical Manager,Provides nursing care including nursing tasks that may be delegated to other roles such as LVN, CHHAs and Caregivers and provide written care plansParticipates in and monitors patient and family care compliance activitiesProfessionally manages/coordinates the hospice plan of careComplete clinical / medical documentation on all patientsParticipates in bereavement support if neededParticipates in and during non-business hours such as evenings, weekends and holidays based upon program-identified needsAppropriately identifies/utilizes resources, referrals such as PT, OT, Hospice, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults.Required Experience:Recent graduate with RN License with at least2 years of Licensed Vocational Nursing (LVN) will be consideredDiploma, Associate, or Undergraduate degree in Nursing.Current and unrestricted Registered Nurse license.Current CPR/BLS certification.Minimum 1 year of recent acute care experience in an institutional setting preferred.Minimum 1 year of experience in wound care, patient teaching, medication management, lab draws, and IV infusion.Experience with Electronic Medical Record Software (Wellsky / Kinnser a plus).COVID-19 considerations:Neogen Care, an essential health industry company, practices all protective measures recommend by the WHO, CDC, state, and county to safeguard our employees and patients from COVID-19 exposure to include: screening, testing, PPE, social distancing.
