For you this means stability and opportunity for advancement.Come and join an Award winning team of Home Health and Hospice professionals!The impact you will make: work with other medical professionals to improve motor skills to our patients.Complete initial and on-going assessments to determine level of functioning, including OASIS assessments at appropriate time points.Execute assessment using procedures such as manual and electrical muscle examinations, functional evaluations, gait analysis, sensory testing, measurements of range of motion, and assessment of development and utilization of human movement.Provide therapeutic treatments and evaluation of equipment needs, as applicable.Instruct patient in use of equipment, as appropriate, such as wheelchairs, braces, canes, and crutches and prosthetic and orthotic devices.Evaluate the home environment and make the appropriate recommendations.Advise, consult, and when appropriate, instruct family and other agency personnel in patient’s therapy program.Participate in discharge planning and evaluation of outcomes of care.Supervise Physical Therapy Assistants and Home Health Aides, as appropriate and document supervision.Prepare and submit all case documentation, including visit notes and progress reports, and required payroll information according to Agency policy.Provide services within the Agency’s catchment area.Additional list of duties and responsibilities is provided in the Job DescriptionWhat you have accomplished:Current, active license as a Physical Therapist by the Physical Therapy Examining Committee of the California Board of Medical Quality AssuranceCore competency per skills checklistAt least one year of experience in physical therapy.Assisted Offers Our Employees a Unique Employment Package:Assisted is one of the leading providers of home health and hospice services with multiple locations in Southern California and Arizona.

Must be able to communicate clearly verbally.

Must be able to read, write and speak English fluently.

Physically active walking, standing, bending, lifting, and climbing stairs.

Must be able to perform fully assisted transfers, with or without transfer devices (if required for the position).

Must not have any back lifting/moving restrictions.

Must provide proof of automobile insurance upon hire and at each renewal and maintain a safe driving record.
