POSITION DESCRIPTION:The Case Manager (CM) is responsible for case management services including assessing client’s/family’s needs, developing, in conjunction with the clients, an Individualized Service Plan (ISP), implementing the ISP, completing quarterly reports on the progress of the clients, providing direct services to family members in strengthening protective factors and addressing the Seven Core Issues in Adoption and co-facilitating support groups for youth and/or families. The majority of work is completed in the field and during times convenient for the clients. The program goal and outcome is that children referred to CB will have permanency and stability in their living situations through adoption when adoption is in the best interest of the child.VI.
