Job Description Under the direction of the Quality Improvement Manager, the Quality Improvement Coordinator is responsible for collecting, gathering, archiving, tracking and generating reports within the quality team.

They are responsible for organizing reports and gathering the necessary information in preparation for analysis by the Quality Specialist and Quality Improvement Manager The Quality Improvement Coordinator works collaboratively with the Quality Improvement Specialists and Quality Improvement Manager to promote and ensure compliance throughout the PACE sites in accordance with CMS.

The Quality Improvement Coordinator reviews and conducts submission audits for tracking of key quality deliverables.

The Quality Improvement Coordinator is accountable for creating a supportive environment with the PACE sites to sustain implemented quality improvements that meet the demands and standards set forth by the California Department of Healthcare Services (DHCS), Centers for Medicaid Services (CMS) and other programs.

This position coordinates with internal and external customers to meet deliverables throughout the year.

Qualifications Current CPR/First Aid TrainingMedical Assistant CertificateHigh School DiplomaAssociates Degree-preferredA minimum of two years of experience working in an outpatient setting, within the geriatric population.

Experience with PACE Quality Data reporting preferred.

Prior experience a minimum of one year with an Electronic Health/Medical System (E.H.R./E.M.R.) EPIC preferred.Prior experience a minimum of one year with incident reporting system preferred Responsibilities Performs ongoing tracking and gathering of required CMS PACE Quality Monitoring & Reporting Data that include Appeals, Grievances, Medication Errors, Falls without Injury, Emergency Room Visits, and CMS Incidents.Conducts ongoing tracking, gathering, archiving and auditing for compliance with the AltaMed’s Infection Control Program.

Deliverables include Hand Hygiene and Reportable Diseases Log.

Generates data reports from multiple systems to facilitate process improvement analysis by the Quality Improvement Specialist.

Gathers data reports to include COVID Cases Reports, Post-Mortem Reports, Appeals, Grievances, Medication Errors, Appeals, Grievances, Medication Errors, Falls without Injury, Emergency Room Visits, and CMS Incidents.

Supports the quality team with developing QI project tools, presentations, agendas, and coordination of QI meetings.

Supports the use of performance improvement tools that sustain PACE sites processes, QI activities, track data gaps, and decrease health care disparities.

Supports QPID in service, process and quality improvement efforts in support of lines of business and programs to ensure standardization and integration between programsGathers demographic, diagnosis, and utilization, clinical quality, and performance data for analysis and reporting Utilizes quality improvement tools and various required platforms to collect, analyze, interpret, report inclusive of presentations and display data that improve processes and patient care.Assists the Quality Improvement Manager with PACE Policy archiving and tracking.

Assists Medical Director of Senior Care Services and Quality Improvement Manager to work with sponsors, regulatory agencies, internal and external stakeholders, to support quality initiative deployment, issues resolution, change/manage by gathering and archiving accurate validated data.

Participates and collaborates with Quality Improvement Manager and QID in preparation for CMS and DHCS Audits.
