The focus is on developing daily living skills, socialization, independence, behavior management and pre-vocational skills.

Key ResponsibilitiesActs as liaison between residents, families, service providers and the community for all DCFS case management domains.Ensure that all intakes are completed in such a manner that meets the needs of the individuals/families and program.Interface and network with outside agencies (Regional Centers, DCFS, school districts etc.) and community programs to create a consistent referral and intake system of collaboration and support.Work closely with mental health clinicians and other case managers to insure that NSP goals and objectives are implemented, monitored, and revised as needed.Ensure that the intake/admission packet is complete including, but not limited to, legal guardianship, Social Security Supplemental Security Income (SSI) funds being transferred to regional center as payee, Authorization for Service, JV220, Education Passport, medical background information, consents and all other documents.Identify appropriate community resources and linkages to support needs and service plan goals and objectives.Required Education, Skills, and ExperienceBachelor’s degree in a related field from an accredited college or university.Minimum two years of experience working as a Case Manager, preferred.A minimum of 1-2 years’ of experience working with at risk severely emotionally disturbed (SED) youth and/or non-minor dependents required.Knowledge of local regulations, state laws, and proceduresKnowledge of social casework principles and methods.Considerable knowledge of the Regional Center and Department of Children and Family Services systems, and Supplemental Security Income (SSI) and Medi-Cal resources.Minimum 1 year of experience making effective use of community resources, establishing and maintaining an effective working relationship with children, families, staff and outside agencies.IND
