She/he ensures that the Triage operations are effective and efficient and performed according to APHCV’s policies and procedures. While the Triage Unit Supervisor directly supervises the Triage Unit staff, she/he provides performance expectations of Triage Operations to Triage Function staff (Triage Function staff are those who do not directly report to the Triage Unit Supervisor but perform Triage function at all and any service locations). The Triage Unit Supervisor provides Triage performance expectation score cards of EHRC and BHC RN Supervisors to Nurse Manager who directly supervises these RN Nurse Supervisors. The Triage Unit Supervisor customarily and regularly exercises discretionary powers in the performance of his/her duties. He/she also provides for safety of employees, patients, and property.The Triage Unit Supervisor assures the delivery of primary care services by also serving as the liaison to outside health care providers and hospitals in the coordination of follow-up care of patients post hospitalizations or post emergency department visits.The Triage Unit Supervisor maintains a close working relationship with the Medical Director, providers, Medical Records Supervisor, and Clinic Manager(s) to support efficient nursing operations.APHCV expects all employees to respond and participate in emergency situations per emergency policies and procedures.APHCV requires all staff to comply with Standards of Conduct and Compliance Program related policies and procedures. Such compliance is part of this position’s performance evaluation.DUTIES AND RESPONSIBILITIES:1. Develop annual goal for Triage Operations for the entire organization (including all service locations) and monitor reports and progressReview the areas of improvement for the year, and submit to Nurse Manager for approval. Include in the goals, APHCV’s overall goals for Triage, if any.Communicate with Triage Unit and Triage Function RN staff at APHCV’s service locationsProvide staff training as needed to achieve the goals.Conduct spot audits of staff to ensure that they are meeting the goals and expectations.Review and plan staffing as part of annual planning and review quarterly if staffing is adequate. 2. Initiates and participates in quality improvement and assurance activities and the development of corrective action plans; implement practice improvements, prepare and review quality of care and productivity performance reports for Nurse Manager.3. Privilege RNs who function as Triage staff to execute Medication Refill function according to APHCV’s policy.4. Management and Supervision of Triage Unit staff at the siteEnsures adequate staffing (daily and weekly) and distributes work accordingly. Make recommendations to Nurse Manager for hire and fire, advancement/promotion and any other personnel change of status of his/her staff. Handle the staff complaints, and discipline her/his staff. Recommend setting and adjusting pay rates and work hours. Supervises and trains the Triage Unit staff to ensure that they adhere to patient safety, quality of care and scope of practice according to the policies and procedures of APHCV. Provides performance evaluations of his/her staff Performs timely follow-up on all incidents and file incident reports according to the APHCV’s Incident Report Policy and Procedures. Promotes a working environment that is conducive to effective team work. Serves as a resource person and problem solves staff concerns in a timely manner.Ensures that the Triage Unit staff maintains efficient flow of patient care from the time of registration through completion of the visitSupervises and ensures adequate medical supplies, types, and appropriate use. Supervises and ensures the correct functioning of patient care equipment, and that mal-functioning equipment is not in use; Works with Nurse Manager when purchasing patient care equipment. Ensures timely compliance with communicable disease reporting.Ensures that the Triage Unit staff observes confidentiality of all patient information provided verbally or in writing to the patient, patient’s family member, and health care provider within or external to APHCV,Ensures that the Triage Unit staff provides best customer service and responds timely and effectively to patient/family complaints,Evaluates training needs of Triage Unit staff at the site, and provides training as needed. Provide input to Nurse Manager and Human Resources Manager for staff training and development.5. Communication and Staff MeetingsConducts staff meetings to update staff on changes and opportunities to improve patient care, posts meeting minutes on SharePoint. Provides timely communication to staff for changes, acknowledgements, and improvement needs. Holds huddle every morning. Ensures that staff attend all of Nursing department staff meetingsHolds staff accountable for reading and understanding information provided to them. 6. Triage Responsibilities Performs patient Triage over the phone and in face to face encounters,Facilitates an effective and efficient patient flow that expedites patient care,Performs a complete assessment according to scope of practice and APHCV polices and procedures,Recognizes signs and symptoms of urgent care needs and rapidly assists patients in obtaining a provider assessment and/or transfer to a higher level of care, Maintains internal controls by timely, effectively and efficiently completing all required documentation,Conducts case management of patients that have been recently discharged from emergency rooms, hospitals and/or had surgical procedures, and documents in EHR and SharePointReport, communicate and coordinate care with Public Health Department/s, Prepares and submits performance and productivity reports for Triage to Nurse Manager and Director of Nursing and TQM,Trains/precept RN staff on Triage Unit functions and policies and procedures, Assists Nurse Manager and Director of Nursing in developing triage policies and procedures for all APHCV sites; and trains staff on such policies and procedures. 7. Dispensary Operations When assigned to dispensary, the RN:Ensures that dispensing duties are performed according to the State of California Nurse Practice Act, regulations set forth by the State of California Board of Pharmacy and APHCV’s Dispensary policy and procedures,Provides appropriate labeling of medications, Ensures that pharmaceutical inventory meets established par levels and that out of date medications are disposed of according to APHCV policies and procedures. Stores medications and medication supplies in a clean and organized environment complying with manufacturer’s recommendations, Ensures that staff comply with infection control procedures when present in the dispensary, Maintains internal controls by timely, effectively and efficiently completing all required documentation, Oversees the duties of all dispensary personnel according to State Board of Pharmacy regulations, scope of practice regulations and APHCV policies and procedures,Maintains a collection of patient instructions for a variety of medications that have been approved by the Director of Nursing, Nurse Manager and Medical Director, Maintains the security of the dispensary by limiting use to approved personnel and securing the dispensary keys at all time. 8. Educationinitiates staff education in-services for nursing staff and other staff based on new procedures and policies and review of current ones as needed, b. Reviews patient care educational materials and recommends changes that align with current clinical practice standards and patient literacy needs.9. Promotes an effective and efficient patient care environmentInitiates and participates in quality improvement activities and the development of corrective action plans; assists in the implementation of practice improvements,Maintains professional demeanor and excellence in customer service,Ensures that patients have needed information about scheduled appointments to promote their understanding and compliance.Initiates and participates in nursing departmental projects,Communicates respectfully and clearly with patients, family members and external vendors whether in person or on the phone, Ensures confidentiality of all patient information provided verbally or in writing by the patient, patient’s family member, and health care provider within or external to APHCV,Supervisor, and Clinic Manager(s) to support service delivery Responds to and provides leadership in emergency situations,Performs all other duties as assigned.10. Professional DevelopmentAttends APHCV assigned meetings and reports relevant information to staff, Attends trainings as assigned by supervisor,Assumes responsibility for own learning needs and communicates training needs to supervisor,Participates in agency wide events as mandated by APHCV’s policies and procedures.11. Performs all other duties as assigned by Nurse Manager or Director of Nursing.QualificationsRequiredCurrent RN license in the State of California, Privileged to fill medication under APHCV’s policy Two years of experience as a registered nurse,Two years of experiences in a nursing leadership position,Recent experience in a clinic or outpatient setting, BLS certified according to the AHA guidelines,Good communication and interpersonal skills, Ability to demonstrate patience and convey a warm, caring attitude toward patients and staff,Willingness to learn new skills,Able to work with people of diverse cultural, educational, social, economic, and linguistic backgrounds,Regular predictable attendance during clinic hours,Ability to work efficiently and accurately with minimal supervision,Able to teach others.HR Procedural RequirementsLegal authorization to work in the United States,A valid California Driver’s license with clean records and personal Automobile insurance (required if staff member would be driving for any APHCV related activities),Completion of Department of Justice background check and fingerprinting,Completion of APHCV New Hire Health Clearance Certification Form and pre-employment physical, including required immunizations. Skills Preferred:Bachelor’s degree in Nursing, Proficiency in Microsoft Office applications,Able to clearly communicate in English and at least one of the following languages: Thai, Khmer, Tagalog, Vietnamese or Spanish.PHYSICAL REQUIREMENTS:Must be able to materially perform the task normally associated with the position including but not limited to: ability to lift up to 45 lbs.
