Supervisor, Social Work

Overview: Responsible for administrative oversight and supervision of direct reports at assigned PACE site that will include masters level social workers, as well as other PACE team members as needed. Promotes effective collaboration and care planning amongst interdisciplinary team members. Responsibilities include, but are not limited to: coordination of routine assessments, care planning, and other related tasks within the scope of practice; assisting with family conferences, community referrals, and interpretation of established policies and procedures, as well as of state and federal regulations; and collaboration with other site leaders to monitor site safety, overall staffing changes, and program budgets. Responsibilities:

Conduct interviewing and hiring tasks, disciplinary counseling as needed, performance evaluations, and respond to PTO and sick leave requests, for all assigned direct reports. Coordinate new hire onboarding and orientation to prioritize essential skills and responsibilities within the first 90 days. Provide leadership and oversight for effective completion of incident reports, grievances, service delivery requests, appeals, and disenrollments to ensure compliance with established notification timeframes. Assist in the coordination and management of satisfaction surveys, HOS-M surveys, and participant rounding as directed. Champion individualized care planning and promote effective collaboration via attendance at IDT meetings. Provide clinical support to MSWs re: identified individual and group counseling needs, as well as crisis intervention as needs arise. Engage in action planning with other site leaders to achieve established quality metrics and assigned program goals. Complete documentation in medical chart in a timely manner and as required. Keep up-to-date on changing rules and regulations of entitlement programs and facilitate communication with governmental agencies such as Medi- Cal, Medicare, and Social Security. Collect and analyze data related to program metrics (voluntary disenrollments, grievances, service requests, etc.) to identify trends and develop improvements for the department. Submit all required reports by the established timelines for reporting internally to QM, or externally to DHCS and CMS. Lead efforts towards audit preparation by continuous observation, feedback, and communication of program and/or staff needs. Consistently adhere to Social Work Code of Ethics, maintain confidentiality, and strive towards service excellence. Conduct monthly team meetings to improve quality of care, enhance skills, address challenges, and discuss best practices. Attend all team and staff meetings, SW Supervisors meeting, Participant Council Meetings, or other meetings as assigned. Continue professional development through engagement in educational opportunities to promote the maintenance and improvement of professional competence. Other duties as assigned. Qualifications:

Masters Degree in Social Work (MSW) from an accredited university is required. Minimum 2 years experience as a Supervisor or Manager required. CPR/First Aid certificate is required. Bilingual: Spanish/English, Chinese/English or another second language is strongly preferred. Minimum of 2 years experience in case management/social advocacy/care planning with geriatric population preferred.
