Essential Job Duties: Meet clients and families in their home setting Develop a plan of care including nursing and support services that meet the client’s needs Determine what care giver team members are appropriate to meet the client’s needs and manage that team ongoing Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.
Requirements: Licensed Registered Nurse.
BSN preferred Prior home care experience REQUIRED Private pay experience preferred 3 years clinical experience Valid US Drivers License
