RN Care Manager

Overview: Advocates is seeking enthusiastic, driven RNs to offer medical supportand care coordinationto those in need.The RN Care Manager provides focused leadership, supervision and support for the care coordination staff.

The RN Care Manager participates in key activities related to the Minimum Data Set (MDS) assessment, completion and submission process.

In addition the RN Care Manager participates in the coordination of behavioral health/addiction treatment services in accordance with the treatment plan, and works collaboratively with the clinical team to manage or co-manage complex behavioral health cases.

As a member of the multidisciplinary team, the RN Care Manager provides consultation, education, and support to team members and provider network regarding co-occurring medical issues related to behavioral health/substance abuse treatment and management.Are you ready to make a difference Minimum Education Required: Bachelor’s Degree Responsibilities: Hire, train, supervise and evaluate care coordination staff.

Maintain consistent communication with staff and conduct regularly scheduled meetings with staff.Complete the Minimum Data Set (MDS) in the enrollees home or other preferred setting.Establish the initial care plan, document enrollment records in E-Clinical Works (eCW).Accurately maintain and adhere to MDS tracking procedures.Follow the client across the continuum of care to ensure care coordination.

Partner with the client, his/her care team providers and supports to ensure that care plan and safety plan are implemented as developed, and make needed adjustments.Provide education to primary care providers on psychopharmacology and diagnosis of mental health issues.

Coordinate all aspects of service delivery with team members, including ensuring access and securing authorization for services.Collaborate with existing providers and other collaterals and deliver services in accordance with clients plan of care.Monitor clients progress in treatment and assist the clinical team with evaluating the need for continued services.Participate in team meetings to ensure effective communication among all disciplines involved in clients care.Identify community resources to develop natural supports for client.Provide on-call coverage, as needed.

Qualifications: BS in Nursing.

Current registration in nursing from the Commonwealth of Massachusetts with minimum of 5 years direct working with adults in a community setting.One year of supervisory experience preferred.Demonstrated knowledge base and judgment necessary for independent clinical decision making.Excellent organizational, time management and problem solving skills.Strong computer knowledge.High energy level, superior interpersonal skills and ability to function in a team atmosphereKnowledge regarding psychiatric rehab and understanding of recovery model.Excellent written and verbal communication.Must hold a valid driver’s license, have access to an operational and insured vehicle and be willing to use it to transport clients.Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.At Advocates, the health and safety of the people in our care, our employees, and the broader community is our top priority.
