Under the supervision of the Program Manager and/or Senior Manager for Mental Health Services and/or the Associate Director, the Incumbent (Outreach & Engagement Case Manager) will serve as the primary representative for the TRC Program during all outreach and engagement events. The incumbent may also perform professional system navigation, service coordination and advocacy services in connection with treatment of mentally ill and emotionally disturbed consumers that have been a victim of or witness to a crime. Incumbent will work with the multi-disciplinary team and assess clients to determine the level of services and assess additional needs. Incumbent will assist consumers and their significant relational figures in understanding and developing solutions to problems that lead to and results from emotional and mental disturbances. S/he will provide hands-on support to victims of crime and serve as the primary liaison to the Victims-Witness Assistance Program (VWAP). S/he will provide trauma informed care best practices. Incumbents of this position will need to have specialized training in treating trauma, collaborating with Law Enforcement, Victims’ Advocates and the District Attorney’s Office. Outreach and Engagement Case Manager will comply with all ethical/legal mandates for reporting. Essential Functions •Serve as primary representative, attendee and/or facilitator for the TRC Program during all outreach and community engagement events. •Serve as an advocate for clients, accompany the client to service locations, and serve as the main liaison between our program and law enforcement, VWAP and outside departments if and as needed. •Communicate with many sources including courts, Los Angeles County Law Enforcement Agencies, District Attorney Offices and Victims Advocate Representative, Department of Children and Family Services and other community agencies. •Provide technical assistance and hands on support to help clients access and navigate services. •Accompany victims to appointments and court appearances. •Develop community resources related to vocational, housing, education, financial and other areas for children, youth, adults and families. •Possess working knowledge of victims’ rights and community-based support groups. •Work in collaboration with the assigned specialists; provide telehealth services and/or travel within the community to meet clients and to assist with completion of necessary paperwork. •Attend weekly Multi-Disciplinary Team meetings and be prepared to present case management or rehabilitation related concerns. •May conduct groups and co-facilitate groups in accordance with the designated evidence-based model. •May assist with the processing of new client referrals and client walk-ins as needed. •Be responsible for assistance with benefits establishment, in particular with VWAP and assistance to all clients and must be familiar with the guidelines of mainstream benefits. •S/he will attend mandatory training related to clinical and non-clinical topics; and collaborate with/accompany the Therapist or Clinical Case Managers where appropriate in the community. •S/he will be the primary person responsible for community outreach to increase awareness and inform organizations, hospitals, law enforcement about the Center and will outreach and develop working relationships with other agencies whose services will be beneficial to clients. •Will participate in all meetings with law enforcement and District Attorney, trainings. •Collaborations, and task force meetings within the community and be prepared to discuss, educate, present or evaluate the nature and progress of presenting issues. •Attend all meetings as they relate to this grant; update Individual Tracking Log regularly. •Document effectively in accordance with established standards of care on a daily basis •Assist with completion of non-clinical documentation. •Adhere to the weekly paperwork deadlines and document in accordance with the best standards of practice. •Will assume other task as assigned and required for this position assigned by the Program Manager and/or Senior Manager for Mental Health Services and/or the Associate Director •Will participate in all mandatory programs, division meetings and trainings (which includes offsite meetings and trainings), as assigned by his/her Program Director, Senior Manager for Mental Health Programs, and/or the Associate Director. •Will maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values. •Represent the Agency in a professional manner at meetings and community events; maintain regular attendance. •Maintain files/records on client services in compliance with HIPAA and CFR42 and other funding requirements for auditing purposes. •Incumbent must maintain regular attendance and will/can be assigned other duties as needed.

Minimum Qualifications – Knowledge, Skills and Abilities Required •Bachelor’s degree from an accredited college or university in Social Work or a related field with two years or related experience or a master’s degree (MSW or MFT) from an accredited college or university in Social Work. •Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. •Ability to communicate effectively both written and orally with collaborative entities; neat and professional appearance •Attention to detai1 and punctuality required, working knowledge of Microsoft Word, Excel, and Internet •Experience working with homeless persons with co-occurring issues (mental/health/disabilities/substances); be able to report (verbal/written) in accordance with the law all legal issues that surface (Tarasoff. child abuse, vulnerable adult abuse, danger to self/01hers and grave disability), professional experience working within a multi-integrated team. •Be able to coordinate the submission of client documentation if unable to work; if in recovery, a minimum of three (3) years of being drug and alcohol free is required. MANDATORY •Verification of Employment Eligibility and Background Check. •Maintain access to reliable transportation that is supported with active insurance and Driver’s License and valid insurance •Be able to work with co-occurring populations that present with any stand alone or combination of mental illness, health conditions and substance related concerns. •Knowledge of the medical model and evidence-based practices. •Bilingual preferred but not required. •Computer literate, with basic knowledge of Microsoft Office Word, and Excel as well as a high level of initiative in keeping current with technological change. •Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner. •Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving. •Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic, and cultural background, and good diplomatic skills. •Good basic clerical skills, including typing, ability to proofread own work, good grammar, and spelling. •Flexible team player. •Excellent attention to detail. •Ability to work very independently and seek supervision when appropriate. •Ability to set boundaries, resolve conflict, and de-escalate issues. •Must be able to pass a comprehensive criminal background check. •Possession of a valid California Motor Vehicle Operator’s License (local travel on behalf of the Agency may be required utilizing company vehicles). •Ability to read and interpret general business correspondence, policies and procedures, referral information, financial information, financial documentation, applicable government regulations. •Ability to write case notes, uncomplicated reports, instructions, and procedures. •Ability to present information effectively and respond to questions from patients, staff referral sources and the general public. •Thorough knowledge of and ability to apply business arithmetic skills accurately and rapidly •Ability to solve practical problems and deal with a variety of concrete variables in situations where standardization may be limited. Ability to interpret a variety of instructions furnished in written, oral, schedule, or diagram format. •Basic math skills. Supervisory Responsibilities: •This position does not have any supervisory responsibilities. Environmental Conditions (Working Conditions) May be exposed to aggressive/hostile clients: Dealing with trauma may trigger personal trauma. There is some responsibility to work in noisy environments where adults are free to talk loud and expressively. Must be able to work effectively and produce quality work under pressure. While driving to trainings or performing other duties there may be exposure to weather conditions prevalent at the time, noise, moving vehicles, high crime rate areas, odors, fumes, pollen etc. Physical Requirements •The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or control; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb, balance, or stoop; rarely to crouch or crawl. The position also requires driving personal and company vehicles. •The employee must occasionally lift and / or move up to 30 pounds. •The employee must have close, color, and peripheral vision and the ability to adjust focus. •The noise level in the work environment is moderated. Mental Requirements The incumbent to be able to perform effectively under any and all of the following •Unpredictable situations, crises, constant distractions, interruptions, uncontrollable changes in priorities/work schedules; able to process information, think and conceptualize.
