Central Standard TimeThis is an anticipated vacancy and will serve as the Admission & Placement CoordinatorBenefits Included for Selected Exempt Service (SES)Health Insurance Premiums $4.17- Single / $15.00- Family -BiweeklyLife Insurance $25,000 (100% employer paid)10 Paid Holidays annually176 hours of annual leave and 104 sick leave annuallyBenefits information can be found at https://www.mybenefits.myflorida.com/DescriptionSubmit referral recommendations to state office for review; coordinate transition meetings with region staff; participate in community placement visits; assist residents in selecting community providers; meet with Sunland management staff as needed; monitor admission and discharge process; request updates from region staff and report to resident team; complete and submit discharge report for each community referral.Coordinate with team and Region staff on specific transfer date; notify state office of discharges and admissions; receive requests for admissions, distribute referral information, communicate with community facility, coordinate meetings between facilities.Ensure resident meets criteria for ICF/IID placement, coordinate admission date.Serve as contact for incoming inquiries regarding admission and discharge; track and monitor internal referrals.Submit requests with supporting documentation to State Office of Authorization for admission; ensure authorization is approved and signed by Superintendent or designee; return signed authorization to State Office.Track referrals and transitions of residents on a spreadsheet; publish on schedule.Review and submit team recommendations to Accounting Department at least annually.Schedule HIV Counselor training or update training annually for social workers; maintain HIV Counselor certification.Coordinate family satisfaction survey; quarterly social services meeting; maintain voter registration data for residents; provide staff training as needed or requested.Monitor and assist in maintaining accurate resident information in iConnect.Submit quarterly resident voter registration report to state office.Coordinate with FLOW Mobile Team for resident Florida identification cards.Assist Unit interviewing applicants for vacant social worker positions.Will serve as back up for the Residential Services Supervisor (RSS) as needed.Bachelor’s degree in a social scienceOne year ofexperience working directly with persons with intellectual disabilities or other developmental disabilities; and experience means providing professional or direct services, either paid or volunteer in a setting that serves persons with intellectual disabilities.
