The Home Care Manager is responsible for overseeing the provision of providing safe delivery of personal care services within their assigned region to the WelbeHealth participants in their homes, including overseeing Home Care Supervisor s scheduling oversight for Home Care Assistants, (HCAs) and managing relationships with external contracted vendors. The Home Care Manager is a member of the WelbeHealth leadership team and facilitates collaborative relationships across WelbeHealth and with external stakeholders to promote the WelbeHealth values, team culture and mission.

Duties and Responsibilities:

  • Oversee the management of the Home Care team that performs activities of daily living
    (ADLs) to WelbeHealth participants, in their homes, including but not limited to, bathing,
    dressing, grooming, toileting, and transferring.
  • Oversee the management of the Home Care team that supports participants with
    Instrumental Activities of Daily Living (IADLs), including but not limited to, laundry, light
    housekeeping, shopping, light meal preparation, and feeding participant s and ensuring
    participants are nurtured, hydrated, and provided with medical escort as assigned.
    Adhere and maintain compliance with the Department of Social Services HCO licensure
    regulations.
  • Provide feedback to agencies/contracted vendors on external caregiver performance
    and participant interactions as needed.
  • Responsible for operating a high-performing home care department ensuring workflows
    and policies are followed at all times.
  • Oversee the effectiveness of Home Care Coordinator assessments and IDT
    participation.
  • Provide oversight or conduct on-sight supervision and instruction to Home Care
    Assistants assigned to participants homes at least as frequently as specified in
    regulations and more often if necessary. Manage the scheduling and tracking of home
    care hours and HCA assignments ensuring efficiency and adherence to participant care
    plans.
  • Work with providers, care coordinators and other members of the IDT to manage smooth
    care transitions between settings (hospital, skilled nursing facilities, home, etc.) and
    deliver high-quality end-of-life care.
  • Manage direct reports including hiring, training, supervising, and mentoring.

Skills and Experience:

  • Minimum of (2) years experience managing home health or home care operations.
  • Minimum of (2) years experience in home care with a frail and elderly population.
  • Nursing knowledge and skills necessary to treat frail, elderly participants and manage
    complex clinical situations.
  • (2) years supervisory experience with demonstrated ability to mentor and develop
    team members.

Medical Clearance:

Required that Team Members must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Additional screening may
be required in order to protect participants and the community from communicable disease outbreaks (e.g., COVID-19).

Regulatory Requirement ongoing:

Must not be on any exclusion or debarment from participation in Federal Health Care Programs at any time and must remain in good standing with government regulators such
as the OIG, (Office of the Inspector General), SAM, (System for Award Management), CMS (Centers for Medicare and Medicaid Services), etc.
