Job Description JR 2468 Enhanced Care Management Care Manager – CalAIM Los Angeles, CA 90004 Salary: Starting at $21.64/hr DOE Full Time Non-Exempt PATH is seeking candidates with case management experience to join our Los Angeles CalAIM program as a Community Support Care Manager. ABOUT THE CALAIM PROGRAM The CalAIM Program services eligible Medi-Cal beneficiaries experiencing homelessness with the goals of improving their health outcomes and decreasing their utilization of high-cost services. The program is comprised of two teams, the Enhanced Care Management (ECM) and Community Supports (CS) teams. The ECM team focuses on coordinating physical health, behavioral health, and community-based services and supports needed by participants. The CS team focuses on taking a holistic approach to linking participants to services that improve their wellness and housing stability through street outreach, system navigation, housing navigation, and tenancy support. WHAT IS A ECM CARE MANAGER? As part of the CalAIM team, the ECM Care Manager guides beneficiaries with complex health needs through the healthcare system by assisting with access challenges, developing relationships with service providers, and tracking interventions and outcomes. Responsibilities of the ECM Care Manager include: Providing all necessary and indicated direct and referral services to members and their families Conducting initial assessments and periodic reassessments of members’ needs Developing patient-focused care plans in coordination with other providers and members Working with medical staff to develop, implement, and coordinate care plans for members with chronic illnesses Coordinating member services with internal and external service providers through regular case conferencing Providing a full range of support to members and providers to ensure their care needs are met including, but not limited to, appointment coordination and scheduling, assistance with prescription refills, and assistance with insurance WHAT YOU BRING We’re looking for candidates that: Are knowledgeable of chronic medical conditions, mental illness, substance use, and homelessness Are knowledgeable of local social and health services resources and how to acquire and effectively use such information Have a basic understanding of public benefits and entitlements Demonstrate excellent case management and documentation skills, including the ability to conduct needs assessments, prioritize member needs, develop action plans, and monitor progress. Demonstrate knowledge of advanced case management techniques such as motivational interviewing. Demonstrate ability to work effectively and professionally with diverse communities Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs Work just as well independently as they do on a team Exercise mature judgement, and are highly motivated, self-starting and proactive Are excellent at communicating, whether in writing or verbally Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment MANDATORY REQUIREMENTS For this role the ECM Care Manager must: Hold a Bachelor’s Degree in a related field and have at least 1 year of case management experience with highly vulnerable populations, particularly those with health, mental health, and substance use issues. Demonstrate proficiency with computers and technology including Microsoft Office (Word, Excel) and electronic health record systems. Be able and willing to work flexible hours which may include evenings or weekends Maintain regular and consistent attendance Have employment eligibility verification Have or be able and willing to obtain CPR/First Aid training Provide proof of full COVID-19 vaccination Have reliable transportation and: A valid driver’s license Auto Insurance The ability to qualify for PATH insurance coverage Successfully complete the following as a condition of hire: Tuberculosis Test Background Screening Drug Test COVID-19 VACCINATION Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19.
