We’re looking for someone to join our team as a Director, Clinical Integration and Social Impact who:

Oversight of design and execution of the clinical integration program to successfully achieve annual goals and priorities, and progress toward long-term strategic plans.

Ensures appropriate focus of activities, integration with cross-functional stakeholders and seamless execution.

Engages community partners to establish the medical/wellness neighborhood model and enhances the population health plan, focusing on prevention and healthy living.

Supports contributions from clinical teams and guides direction of cross-functional teams to ensure that work output leads to enhanced clinical efficiency, appropriateness of care, and successful achievement of payer-focused initiatives.

Mentors and develops staff members and supports the overall growth of knowledge and skill with clinical integration.

Essential Functions:

  • Establishes appropriate interventions throughout the care continuum.

    Tracks patients upon discharge, addresses follow-up care, leverages Integrated Care (IOPCM and CC) and LW as needed, works with the clinic to segment population and assures appropriate care plans are in place.

  • Addresses referral needs both inside and outside the AH network.

    Establishes a strong communication system to share data, works with local community organizations to address the needs of upstream activities.

    Aids housing efforts.

    Assists the full integration of behavioral health services throughout all service lines, collaborates with community partners [e.g.

    school clinics, external mental health facilities] to assist with behavioral health delivery and availability.

    Works with the corporate office and the population health effort.

    Attends system meetings to provide clinical oversight and aid the use of data from Cerner and other sources to provide impactful interventions.

    Looks to improve the financial viability of the PHC capitated beneficiaries.

    Works with the capitation team to repatriate members who are out of network, identifies service expansion opportunities and presents to senior management when identified.

    Maintains agreed upon budget.

  • Performs other job-related duties as assigned.

You’ll be successful with the following qualifications:

Education:

  • Bachelor’s Degree or equivalent combination of education/related experience: Required
  • Master’s Degree: Preferred

Work Experience:

  • Seven years experience: Preferred
  • Five years experience in case management work in a clinical setting: Preferred

Required Licenses/Certifications:

  • RN or LSW in the state of practice: Preferred
  • Case management certification: Preferred
