CNS helps people with brain injury find hope, independence and a meaningful life and you can play a key role in that journey as a Case Manager.The Clinical Case Manager is concerned with and, under the supervision of the Director of Rehabilitation, responsible for the development, implementation and management of all clinical aspects of Centre for Neuro Skills.

He/she is an integral part of the overall decision making process bridging both clinical and administrative matters involving both the clinic and residential sites.

The six core components of clinical case management include: processes and relationships, health care management, community resources and support, service delivery, psychosocial intervention; and rehabilitation casemanagement.

Work is full time during the weekdays and requires some on-call and overtime work.

Work is performed primarily at the clinic with time also spent at the residential site and minimal time in the community.In general, the Clinical Case Manager position requires a high level of accuracy and dependability in decision making, coordination of patient programs, documentation, adaptability to a variety of situations, strong organizational skills, an ability to evaluate situations as they occur and overall management of the clinical aspect of Centre for Neuro Skills.For over 35 years, CNS has provided post-acute medical treatment, rehabilitation and disease management, and offers residential, inpatient, outpatient and assisted living programs.

The essential activities of the clinical case manager include the following:a.

Collects in-depth information about the patient’s situation and functioning to identify individual needs, and assessment of thatinformation to develop a comprehensive case management plan.

Information will be gathered and assessed from other relevantsources as well.b.

Plans the specific objectives, goals and actions designed to meet the patient’s needs.

This plan should be action-oriented and time-specific.

Establishes, communicates, and facilitates initial program issues and goals with appropriate designated staff via the patientpreadmission meeting.

Communicates with external stakeholders and families to ensure coordination of services.

Requires excellentcommunication skills.c.

Implements the specific case management activities and/or interventions that will lead to accomplishing the goals set forth in theplan.d.

Coordinates, organizes, secures, integrates and modifies the resources necessary to accomplish the goals in the plan.

Must haveknowledge of legal and medical aspects of case management.e.

Monitors the plan and its activities and/or services through the ongoing process of gathering sufficient information from all relevantsources to enable the case manager to determine the plan’s effectiveness.f.

Evaluates, at appropriate intervals, the plan’s effectiveness in reaching the desired outcomes and goals.

This process might lead toa modification or change in the case management plan in its entirety or any of its components.2.

Supervises and directs all aspects of patient programs, including development, assessment, implementation, coordination, and communication via biweekly staffing, monthly conferences, patient contacts, and post-admission staffings.

Promotes responsiveness to patient strengths, abilities, needs, and preferences.3.

Participates in weekly program review and supervision sessions that may include the President, Executive Vice President, Vice President(s), DOR, ADOR, Residential Rehabilitation Manager, Residential Treatment Coordinator(s), Residential Rehabilitation Programming Manager, Nurse(s) and other staff as appropriate.4.

Schedules and facilitates conferences for each patient on a regular basis, usually every 30 days.

Conferences may include the carrier, family, attorney, physicians, patient and other stakeholders.

Coordinates interdisciplinary reporting.

Produces a report of all information presented at the conference.

Requires excellent communication skills.5.

Acts as liaison between the facility, patient, patient’s family, carrier, and other parties.

Available to family at any time for questions and/or assurance.

Seeks input from family, person served, and other relevant stakeholders.

Orients patients to the program using handouts, admission forms, website and tours of the facility.

Promotes the participation of the patient in plans and goals on an ongoing basis.

Acquires authorizations from carrier for services and special therapeutic needs, i.E.

equipment, non-routine medication and appointments.6.

Produces high quality reports and/or documentation of patient program issues.

Disseminates information regarding patient and his/her program to appropriate persons, including the patient, family, carrier, therapists, referral sources, discharge/transfer facilities, etc.

Participates in program outcome, record review, performance improvement, etc.7.

Promotes integration of all disciplines by encouraging dialogue, inservices, and inter and intra-departmental education.8.

Upon or before patient discharge, identifies facilities, resources, and physicians in the designated geographical area.

Facilitates discharge and transfer arrangements.

Handles paperwork regarding the discharge.

In conjunction with the Aftercare Coordinator, may perform follow-up for patients by telephone.9.

Provides for the safety of patients, visitors, staff and self when interacting with patients who may have behavioral outbursts, which can include yelling, exiting, hitting, kicking, biting, spitting, scratching, and throwing objects.a.

Must be able to recognize patient escalation, via verbal, physical and visual cues.b.

Must be able to attempt to verbally and physically de-escalate patient using Behavior Modification techniques according to eachpatient’s program, which may include Professional Assault Crisis Training (Pro-ACT) such as evasive self-defense (ability to get out ofthe way) and controlling self-defense (restraints).c.

Physical intervention and restraints can include calling for help, lifting, standing, walking, running, stooping, bending, kneeling,squatting, carrying, pulling, pushing and assisting with the restraint of patients weighing up to 200 pounds or more.10.

Performs toilet and chair transfers without assistance for patients up to 150 pounds utilizing approved transfer techniques.

Transfer techniques include standing, walking, stooping, bending, kneeling, squatting, lifting, carrying, pushing and pulling.

Patients requiring moderate to maximum assistance and weighing over 200 pounds are lifted and/or transferred by two persons.11.

Uses special equipment as required by each patient including, but not limited to wheelchairs, tracheostomy tubes, gastric feeding tubes, and external catheters, at any given time according to the outline of each patient’s program.

May require fine motor manipulation, lifting, reaching, carrying, pushing and pulling when working with equipment.12.

Maintains professional and ethical relationships with staff, patients, patient families, and other visitors.13.

Attends staff meetings, in-services, and all scheduled shifts.14.

Conducts on-site or in-house evaluations for potential patients.

Performs bed, bath, toilet and chair transfers without assistance for patients up to 150 pounds utilizing approved transfer techniques.

Transfer techniques include standing, walking, stooping, bending, kneeling, squatting, lifting, carrying, pushing and pulling.

Patients requiring moderate to maximum assistance and weighing over 150 pounds are lifted and/or transferred by two persons.3.

Shares responsibilities for on-call duties which may include carrying a cell phone, responding to emergency telephone calls, decision making in non-crisis and crisis situations and going to the clinic or residential site at any hour on any day.4.

Performs appropriate public relations activities relating to potential patients, current patients, and general CNS related interests.5.

Participates in the interview and selection process, discipline and evaluation of therapeutic staff.6.

Promotes by example and through education the CNS philosophy to all staff.7.

May screen referral calls and performs follow-up as appropriate on referrals.

May coordinate admission of patients between the patient, family, carrier, attorney and prior facility.

Requires extensive use of telephone as well as written communication via fax machine and letters.8.

Performs therapy sessions as needed.

When this occurs, takes on duties of therapist (see specific therapist job description).9.

Transports patients as needed.10.

Performs other duties as assigned.SPECIAL SKILLS UTILIZED, EDUCATION LEVEL, TRAINING1.

Bachelor’s degree in a related field.

Master’s or licensure/credential required.2.

CCM preferred.

Successful completion of CCM required upon meeting eligibility guidelines.3.

Minimum of 3-5 years’ experience in rehabilitation or related field.4.

Demonstrate a capacity for and interest to absorb a global knowledge of medical and clinical rehabilitation aspects relating to traumatic brain injury.5.

Experience in which supervisory abilities have been demonstrated.
