SUMMARY OF POSITION Provides case management services for an assigned caseload. Works closely with consumers and/or their families. Communicates with staff, vendors and the community. Organizes and resolves work-related situations. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. • Contacts assigned consumers and documents; contacts according to established SCLARC Standard Operating Procedures (SOPs) and policy, and completes required documentation. • Prepares case transfers as needed. • Attends all unit, division, supervision, staff and in-service training meetings as required. • Attends all Planning Team meetings at primary programs and any other consumer related special meetings/hearings (e.g. SCAN, court, IEPs/IFSPs staffings, mental health, governmental/community agencies, placement, etc.) as required. • Handles officer-of-the-day (OD) duties as required. • Handles confidential information and adheres to Welfare and Institution (W&I) codes. • Consults with appropriate specialists regarding the service needs of consumers. • Maintains effective working relationships with top management, peers and staff. • May act as Program Manager in his/her absence. EDUCATION and/or EXPERIENCE Bachelor’s degree (B.
