Responsibilities
The Stroke Program Coordinator will work collaboratively with MD, nurses and interdisciplinary teams to provide professional,clinical nursing and technical knowledge, skills and expertise required for the provision and continuity of comprehensive neurological and stroke patient care. Assist with research, development and application of evidenced based nursing practice in the clinical area,patient and staff education and development and adherence of policies and procedures and overall plan of care. He/she functions as an expert clinician, consultant, educator and leader in the care of the stroke patient population.
- Direct clinical practice includes expertise in advanced assessment of stroke patients, implementing nursing care related to care of stroke patients and evaluating outcomes.
- Demonstrate effective verbal and legible written communications and interpersonal skills with associates, community and other customers. Provide accurate and helpful information and instruction to departmental and hospital customers. Accurate listening skills while checking, understanding with appropriate follow through on request for information and/or services.
- Works collaboratively with the health care team to ensure that both practice and professional standards are identified and maintained in the delivery of patient care and mutually determined patient outcomes are achieved. Ensure Stroke Program meets state, national and Joint Commission standards.
- Demonstrate appropriate time management skills.
- Utilizes the theories and principles of adult education to foster the educational development of patient, families and staff by identifying their educational naeeds, planning for their education, assisting in the development of educational materails for staff, patient and families and coordinating and presenting educational programs and conferences in the care and management of stroke patients.
- Knowledge of hospital and departmental infection control plan, policies and/or procedures, including standard precautions,handwashing procedures,prevention of exposures to blood-borne/other pathogens and needle stick injuries,handling and disposal of sharps, reporting of exposure incidents. Knowledge and application of individual role in external/internal disaster and hospital emergency process.
- Knowledge and application of quality improvement for own work and for departmental processes. Coordinate performance improvement program initiatives including outcome evaluation.
- Writes, reviews and implement physician orders within scope of practice in the care and management of stroke patients. Provide clinical resources and consultation to staff regarding stroke patients with complex clinical needs.
- Develop and implement initiatives in coordination with service line Medical/Nursing Director of the Stroke Program. Works with service line Program Directors in promotion and marketing of stroke program.
- Attends and participates in required stroke/sepsis meetings.
- Represent and promote stroke and sepsis services to hospital staff, volunteers, patients and visitors through positive attitude and effective communication skills. Exhibits professional conduct and demeanor during all interactions.
- Collaborate with Staff Education Department to develop, implement and evaluate basic and advanced curricula related to stroke care. Works with Nursing Directors to implement formal and informal nurse education and training regarding stroke care.
- Review and analyze stroke and sepsis data collected, outcome monitoring and trend reporting surrounding stroke management issues.
- Monitor compliance with the use if evidence based practices in the care of stroke patients.

Qualifications
- Minimum of two years nursing experience, including care of stroke patients.
- 1 year experience in managerial or program development preferred.
- Working knowledge of performance improvement, licensure and accreditation requirements.
- Demonstrate good capacity for teaching, organizing, planning and development.
- Strong critical thinking skills.
- Strong interpersonal communication and customer relation skills.
- Graduate of accredited RN school of Nursing. BSN required
- Strong knowledge of Microsoft office, Outlook and Internet required.
- Current CA RN License, BLS, ACLS
