Position Summary: Responsible for coordinating medication/health needs of our clients

Salary Range: Min $100,000 | Mid $110,000 | Max $120,000

Essential Functions & Responsibilities:

  • Provide ongoing assessment of clients’ physical health symptoms and clients’ response to treatment. Make appropriate changes to treatment plan to ensure immediate interventions are provided in response to changes in medication/health.
  • Provide management and administration of medication, in conjunction with team psychiatrist.
  • Provide medication/health education to clients and families.
  • Focusing on issues related psychiatric medications and physical health, provide direct clinical services to clients on an individual basis in the office and in community settings.
  • Provide service and coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment teams (ITT).
  • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITT.
  • Educate and support clients’ families and advocate for clients’ rights and preferences.
  • Conduct holistic and comprehensive medical assessment along with other team members.
  • Act as liaison and consult with community agencies and families to maintain coordination in the treatment process.
  • With other RN’s, participate in on-call rotation for crisis intervention/support 7 days/wk, 24hrs/day.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Hire and supervise all registered nurses within the program.
  • Attend and participate in all appropriate leadership meetings.
  • Primary clinician for medically intense clients.
  • Oversee medication support at the Residence.
  • Develop and implement training which focus on nursing related topics.
  • This position reports to Team Psychiatrist and PACT Clinical Director.

Physical Demands and Work Environment:

  • Physical Demands: While performing the duties of this job, the employee is occasionally required to walk, sit, drive a vehicle, use hands to type, handle or feel objects, reach with hands and arms, bend, walk up and down stairs, talk, hear, and see. Employee must be able to occasionally lift/move up to 50 pounds.
  • Work Environment: Most work is performed indoors.

Requirements:

Qualifications Required:

  • Registered Nurse with California License.
  • Minimum one year of psychiatric nursing experience.
  • Minimum one year of experience in a clinical psychiatric environment.
  • Excellent organizational skills a must.
  • Good communication and interpersonal skills.
  • Ability to prioritize and multi-task.
  • Dependability/Responsiveness.
  • Must have skills and competencies to establish supportive trusting relationships with persons with severe and persistent mental illnesses as well as family and other members or the clients’ network
  • Must have valid driver’s license, dependable automobile, active auto insurance and a clean driving record
