ConcertoPACE is seeking an enthusiastic Registered Dietitian who will:
* Evaluate and provide for the nutritional needs of each participant and supply access to additional food services.
Collaborate with the adult day center (ADHC) staff to monitor and maintain the meal quality provided to the participants.
Serves as an active member of the Interdisciplinary Team (IDT) to coordinate care for each participant.
* Complete participant initial and annual nutritional assessments and reassessments as needed.
* Assist participants with community food resources including mobile meal delivery programs, congregate meal sites, community food banks.
Collaborate with ADHC staff to meet nutritional needs, special diet adherence, and addresses nutritional concerns for the participants.
* Implement nutrition policies and procedures and conducts food safety checks in dining service environment.
* Assist with the compliance and administration of the nutrition program, including staff training, menu review, and monitoring of meal service.
Oversee menu planning, and meal delivery program to ensure all regulatory guidelines are met and participants receive the correct diet plan.
* Develop and maintain relationships with contractors and staff in contracted facilities to foster communication around nutrition care and interventions.
* Monitor weights, medical diagnosis, changes in chewing, swallowing, hydration, mood changes, intake, other health and environment changes and report changes to IDT members.
* Provide nutrition education to participants, caregivers, and ConcertoPACE staff.
* Support outreach and enrollment activities by providing informational seminars on area of expertise to prospective participants.
Requirements
* Bachelor’s Degree from and an accredited college or university in food science, nutrition, and/or dietetics is required.
* Certified by the Commission on Dietetic Registration (CDR) as a Registered Dietitian.
* Minimum of 1 year experience in long-term care preferred, one year experience working with a frail or elderly population required.
* Comprehensive knowledge of clinical nutrition and therapeutic diets.
* Ability to assess participant’s nutritional needs.
* Compliance with all applicable federal, state, and HIPPA regulations.
* Skilled in medical terminology, including knowledge of the appropriate documentation for completing written assessments.
* Must be able to lift/carry up to 25 pounds.
