Health RIGHT 360 is a non-profit organization dedicated to helping clients in their recovery process.
Our program focuses on assisting men and women in the criminal justice system to successfully transition into society.
Outpatient Care Coordinators are responsible for assessing clients needs and strengths in support of the health and recovery.
Through counseling in individual and group settings, delivering curricula in accordance with program specific requirements and case management, Outpatient Care Coordinators assist clients in navigating systems of care while providing a supportive treatment environment.
Substance Use Disorder Counselor provides clinical services to a caseload of 18 adult male/female from the criminal justice system.
The services are included but not limited to intake, assessment, treatment planning, individual and group counseling, psycho education, relapse prevention, crisis intervention and case management.
Key Responsibilities
Facilitates individual counseling sessions with each caseload client.
Provides learning experience opportunities and offers clinical.
Facilitates group counseling sessions and community meetings and teaches classes on various treatment-related topics.
Performs Documentation Responsibilities:
Assessments, treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Performs crisis intervention and communicates with the treatment team as unforeseen situations arise.
Education and Knowledge, Skills and Abilities
SUD Certification or SUD registered with two years of minimum work experience.
Experience and interpersonal skills working with culturally and socially diverse community groups including drug/alcohol, HIV and hepatitis C impacted and affected individuals and groups, and the undeserved.
Computer Literacy: skilled with Electronic Health Records.
Able to work independently and collaboratively with a team.
Strong independent problem-solving skills and organized functioning.
Actively models th highest ethical standards; responsible, maintains confidentially and appropriate boundaries.
Applicant will need to pass a basic computer test and a CDCR clearance.
