The diabetes educator provides education for clients of all ages with diabetes according to the individualized needs of the client.
This process involves a health and education assessment, development, implementation and evaluation of an education plan, taking into account psychosocial, economic, and language barriers.
Duties are performed virtually, in a classroom or individual outpatient setting in collaboration with a primary care team.
Professional development, clinic integration, and community outreach is an integral part of the role.

ESSENTIAL FUNCTIONS
• Actively supports and incorporates the mission and core values into daily activities.

• Performs comprehensive client assessment, including medical history, health service access and utilization, risk factors, current diabetes knowledge, skills, attitudes, barriers to self-management with emphasis on socioeconomic, cultural, and linguistics resources.
• Develops, implements, and evaluates individualized client education plan that reflects the patient’s assessment and on-going self-management capabilities.
• Maintains confidentiality of all information related to patients, medical staff, and as appropriate, other information.
• Accurately and concisely performs client and program documentation to meet regulatory and reimbursement requirements.
• Consistently demonstrates and incorporates principles of safety and infection control into daily activities as outlined in Environment of Care, Infection Control and Exposure Control manuals.
Maintains knowledge of work-appropriate aspects of Environment of Care program and attends EOC skills training.
• Participates in regular staff meetings, inservice trainings, and on-going program development.
Collaborates with regular staff in a spirit of teamwork to evaluate trends in diabetes education and help the program maintain consistency with national diabetes education standards.
• Participates in community outreach projects focused on diabetes education according to specific talents and interests.
• Cross-train in all aspects of diabetes education as appropriate for the integrity of the program.
