HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County”s effort to address and combat homelessness in the communities residing within the boundaries of the County”s eight Service Planning Areas (SPAs).The Countywide Benefits Entitlement Services Team (CBEST) is a program under DHS” HFH and provides targeted benefit advocacy services to assist individuals, who are homeless or at risk of being homeless (e.g., individuals, families, children, Veterans, etc.) who have complex health and/or behavior health conditions, high utilizers of public services (e.g., Los Angeles County Departments of Mental Health, Public Social Services, Children and Family Services, Military and Veteran Affairs, Sheriff; Los Angeles Homeless Services Authority (LAHSA), in obtaining sustainable income through government programs such as Supplemental Security Income (SSI), or Social Security Disability Insurance (SSDI), Cash Assistance Program for Immigrants (CAPI), and/or Veteran Disability benefits.POSITION OVERVIEW The Assistant Staff Analyst, Health position (also referred to as ”position”) will manage, oversee, and directly supervise the following four CBEST”s teams (also referred to as ”team”) as below:Outreach Team: Veterans and JailsCentralized Assessment TeamApplication TeamThe Assistant Staff Analyst (also referred to as ”position”) will be responsible for one of three core sets of responsibilities: establishing/maintaining relationships with referral partners (e.g., Los Angeles County health systems, local homeless systems, Los Angeles County jails, Department of Children & Family Services, veterans system, etc.) across target areas to coordinate ongoing CBEST client referrals (also referred to as ”referrals”); processing all new referrals, ensuring client is placed at appropriate CBEST step(s), and coordinating all client services needs accordingly; and/or managing a team of staff completing disability benefits applications.
This position will also provide support to HFH program staff by performing an array of administrative/professional tasks and/or duties in support of HFH-funded programs and/or contracted services.
The position will function under the close guidance of HFH executive leadership and/or management personnel (also referred to as ”HFH personnel”) as required.
The position will require travel to all eight Los Angeles County Service Planning Areas to attend various meetings with HFH personnel, other County agency personnel, HFH community/contracted agencies, etc.
and/or attend/participate at off-site HFH related meetings, and conferences, etc.
as applicable.
Core functions of the position will manage, oversee, and directly supervise the following three (3) teams:Outreach Team (Los Angeles County Jail System and Veterans)This team will be responsible for establishing/maintaining relationships with referral partners serving justice-involved individuals and/or veterans (e.g.
Los Angeles Sheriff Department (LASD), Office of Diversion and Reentry (ODR), Correctional Health, Whole Person Care (WPC), Veterans Administration (VA), etc.) across the re-designed target areas to coordinate, support, and maximize the receiving and processing of referrals.
This team will also facilitate presentations/trainings on HFH criteria and referral eligibility to increase CBEST client referral volume.
The ASA on this team will also be required to successfully clear a background check with Los Angeles Sheriff”s Department (LASD) in order to enter the county jail facilities.Centralized Assessment Team (CAT)The Administrative Assistant II (also referred to as ”position”) will be responsible for providing an array of administrative, technical assistance, and specialized functions.
The position will perform overall functions under HFH executive leadership, management, and/or administrative personnel (also referred to as ”HFH personnel”) as required such as: managing, processing and moving CBEST client referrals (also referred to as ”referrals” or ”clients”) through the various steps of the CBEST program.
Specifically, the position will work directly with clients, ensuring clients are properly served and directed to the appropriate next step within the CBEST program, monitoring case progress, communicating with clients, Legal Services providers, Los Angeles County Department of Public Social Services (DPSS), Los Angeles County Department of Mental Health (DMH) and/or Department of Health Services (DHS) clinicians, Social Security Administration (SSA), and SSA”s Disability Determination Services (DDS) office regularly and as needed.
The position will report to HFH personnel as applicable.Application TeamThis team is responsible for developing, completing, and submitting all initial client SSI, SSDI, CAPI applications that have been approved by the CBEST CARES Team and DMH CARES Team for submission to the Social Security Administration (SSA), and/or the Los Angeles County DPSS as applicable.
Additionally, this team will be tasked with completing reconsiderations and hearings requests with SSA or DPSS if a client”s initial application is denied.
This team will work directly with clients to complete applications, coordinate the retrieval of medical/mental health treatment summaries and dispositions from the CARES team, collaborate with CBEST legal services providers on quality review of applications, regularly communicate with SSA, DDS, and/or DPSS once applications have been submitted, and ensure deadlines are met, documents are submitted, and the client is aware of any necessary follow-up in order to ensure greatest likelihood of approval for disability benefits.Duties and responsibilities include, but are not limited to, the following:Oversees, establishes and maintains relationships with referral partners (e.g., Los Angeles County Departments of Mental Health, Public Social Services, Children and Family Services, Military and Veteran Affairs, Sheriff; LAHSA; community-based organizations; HFH contracted providers; etc.) across target areas to coordinate and support the receiving and processing of referrals.Works closely with the veteran and justice-involved entities (e.g., Los Angeles County Jails, Men”s Central Jail, Pitchess Detention Center, Twin Towers Correctional Facility, Century Regional Detention Facility, and North County Correctional Facility) to establish and improve upon referrals partnerships; engages with referring partners to provide updates on client status as needed.Compiles reports on new partnerships with guidance and instructions from CBEST Management and/or Process Improvement Teams.Maintains roster of referring partners; facilitates presentations/trainings on HFH criteria and referral eligibility to increase the referral volume.Maintains accuracy and compliance with HFH reporting and data requirements; prepares and/or assists in the preparation, provides data entry/input, compilation, and data retrieval, analysis on various HFH databases, spreadsheets, documents, and reports, etc.Communicates regularly with referring partners, CBEST contracted partners, Social Security Administration (SSA), Disability Determination Services (DDS), and HFH staff as needed to support client needs, track application progress, ensure proper client service connection, etc.Ensures clients are connected to an array of CBEST services, including case management, in accordance with established CBEST step processes; establishes timelines for client services and completes follow-up accordingly.Provides on-going technical assistance on complex issues to staff, partners, and contract agencies; assists in the analysis of problems and identification of solutions; assists in project modifications as needed to achieve project goals and objectives.Develops, prepares, and/or modifies reporting correspondence to internal and external entities as needed; may respond to (e.g., verbally and/or in writing) to an array of requests for information specific to the CBEST program and/or IT activities.May supervise an array of clerical, programmatic, and/or administrative personnel as needed; may act as lead on projects, meetings, etc.
