Senior Social Worker (HUD-VASH Liaison)

The Greater Los Angeles VA Healthcare Network is recruiting for a Senior Social Worker (HUD-VASH Liaison) within its Housing and Urban Development
– Veterans Administration Supportive Housing Program (HUD-VASH) in the Community Engagement Rehabilitation Service (CERS).

This position is responsible for monitoring the HUD-VASH contract providers for clinical and administrative compliance during their course of providing Veterans with direct services and resources leading permanent housing.

( “The Senior Social Worker will provide social work service to the homeless Veteran population in addition to accessing VA medical, mental health, substance abuse, and other specialty clinical providers, in addition to community providers, to collaborate in the care of Veterans.

The major duties and responsibilities include but are not limited to: Monitoring compliance with the standards outlined in the HUD-VASH Contract and is the primary point of contact for the HUD-VASH Contractors regarding any questions or issues related to the contract.

Reviewing contract agency quality assurance/quality management procedures to ensure quality of care, social worker will review 100% of veteran’s charts via case conference or chart review monthly.

Obtaining Contracting Officer Representative (COR) I and II certification and serve as the COR (government’s technical expert) for a contract, functioning under a delegation from the contracting officer.

Reading the contract in its entirety, including all references and attachments, upon hire and upon award, renewal, or modification of the contract and understand how to monitor contractor performance as outlined in the Quality Assurance Surveillance Plan QASP.

Coordinating and participating in program site inspections utilizing regulations outlined in the contract as guidance.

Monitoring case management services provided to a special patient population that demonstrates highly complex psychosocial, health or mental health problems.

Providing crisis intervention consultation with contractors as needed.

Establishing and maintaining effective therapeutic relationships with Veterans and their families when appropriate.

Providing consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans.

Participating as a team member of the CERS HUD-VASH Department and will adhere to all applicable CERS and HUD-VASH SOPs.

Work Schedule: Monday thru Friday, 8:00am to 4:30pm
