AbilityPath: Family Resource Coordinator

AbilityPath is hiring a Bi-lingual Coordinator for our Family Resources Team The FRC Coordinator works with agencies in the San Mateo Communities, to generate referrals for children eligible for early intervention services or prevention resources referrals and facilitates the efforts of the Family Resources Center.

Candidates must be a parent or family member of a child or adult with developmental disabilities or special needs.

Our organization provides a well-rounded career experience that includes job training and development opportunities, employee recognition programs, paid time off, and other benefits creating a total compensation package.AbilityPath offers a comprehensive benefits package for our full-time employees which includes medical, dental, 401(k), and moreWhat we Value:Passion for the missionAwareness of, sensitivity to, and competence in supporting people with developmental disabilitiesEffective communication, thoughtfulness, ability to adaptPassion and awareness for early child developmentAbout the Role:AbilityPath will support the coordinator to accomplish a variety of goals upon hiring, these include but are not limited to:Coordinate and perform outreach activities to families with children with special needs both virtually and in personTrack all case notes and outreach efforts in ALICE database in a timely mannerOutreach to community providers to promote referrals to the Family Resource Center and track in ALICECollaborate with Help Me Grow San Mateo County (HMG SMC) team and accept warm referrals from HMG SMC teamAssist families at the time of transition from Early Intervention services to School District ServicesContinue and expand parent support activitiesAnswer warmline calls, emails, and web inquiries and provide resource and referral information to parents/caregiversPlan parent and professional trainings and develop an annual Training Calendar to promote trainings to community via FRC website, HMG SMC website, etc.Actively engage and participate in team meetings including: Case Review Meetings, Staff Meetings and Reflective Practice Meetings Education and Work Experience:Any combination of education and experience that would likely provide the required knowledge, skills, and ability is qualifying, unless otherwise specified.Education in preferred in child-development, social work, psychology or related fieldRelevant work experience with families and children birth through fiveBilingual in English/Spanish or other high-frequency languages in San Mateo County such as Portuguese, Mandarin, Cantonese or other Asian/Pacific Islander languages.
