Job Summary:
The Transplant Coordinator is responsible for the planning and coordination of all aspects of care for patients throughout the continuum of transplant services. The Transplant Coordinator will oversee donor offers and coordinate with all interdisciplinary services to determine viability.
Primary Duties and Responsibilities:
• Serves as the oncall coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff.
• Communicates with the Organ Procurement Organizations, exhibits knowledge and ability to
facilitate the transplant process when organ becomes available to the transplant candidate.
• Demonstrates knowledge and skill in managing donor and transplant recipient care
• Responds to patient calls and clinical concerns and appropriately documents in patient’s medical record.
• Collects/documents data to meet clinical, regulatory (OPTN/UNOS/MCS), financial requirements
• Participates in the development and implementation of practice guidelines, protocols, pathways, performance
improvement and research activities.
•Provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan.
• Identify safety concerns and institute measures to provide a safe patient care; and provide appropriate assessment, intervention and evaluation of patients with consideration of clinical and developmental milestones in the patient population.
• Identify the ability of the patients to follow directions/instructions and give consent.
• Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community. Represents the Medical Center at regional and national meetings/conferences.
• Maintains competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds.
This is an 24hr oncall position.
Educational Requirements:
BSN required
Licenses:
Current California RN License
Current BLS
Specialty Certification upon hire or within 1 year
Experience:
3 years Critical Care or Acute Care Nursing experience required
Physical Demands:
Perform continuous operation of a computer for long hours.
Perceptive acuity and physical ability to perform all direct and indirect patients care functions including walking, sitting, standing, bending, and light lifting required.
Stand statically in one place to perform a function without changing location.
