The Nurse Practitioner is responsible for serving as the primary care provider for SNF residents in collaboration with the Medical Director and Director of Geriatric Services. In this position, you will conduct monthly rounds, alternating residents for review with the physician. You will also conduct annual history and physical examinations, including routine health screening and preventative health care within the long-term care facility. You will also be responsible for managing chronic conditions, evaluating acute problems and changes in health status, as well as referring residents for specialty testing and services and to specialty providers. You will be able to rely on and collaborate with a strong interdisciplinary team of colleagues. Requirements include: BSN and MSN degrees Current CA RN License National certification as adult/family and/or gerontological nurse practitioner Current furnishing license and DEA number BLS certification 2 years’ experience as a Nurse Practitioner in geriatrics Experience working in a skilled nursing facility or long-term care setting preferred MPTF offers competitive compensation and perks for full-time and part-time (24 hours per week) employees.
