Reporting directly to the Chief Nursing Officer and collaborating closely with a tenured CFO, the Nurse Director of Case Management plays a pivotal role in reducing hospital metrics such as average length of stay and hospital readmission rates. In this leadership position, the Case Management Director will be responsible for overseeing and coordinating the Case Management department’s operations, ensuring the appropriate allocation of resources while maintaining a focus on patient-centered care.
Key responsibilities include developing and implementing case management strategies, protocols, and initiatives to enhance patient outcomes and promote efficient utilization of resources. The Nurse Director Case Management will collaborate with physicians, nurses, and other healthcare professionals to facilitate effective care transitions and ensure a smooth continuum of care. Additionally, the Case Management Nurse Director will provide guidance and support to the Case Management team, ensuring compliance with regulatory standards and best practices.
