Your contributions to our success requires you to be valued as a leader in your field of expertise, and as an expert in the complex issues that may face an individual living with HIV. As a valued care team member and through your contributions, you would find meaningfulness and purposefulness by also adding value to our Mission, staff, and patients. The Managed Care Social Worker is considered an expert in providing intensive case management for individuals, in collaboration with the RN Case Managers. Drawing upon your experience and skills, you would be essential in performing psychosocial assessments and generating referrals for clients in the arenas of mental health care, substance abuse services, housing, legal assistance, immigration, and linkage to community organizations.
The Managed Care Social Worker collaborates with the client’s Primary Care Provider and RN Care managers on a regular basis. Your feedback at multidisciplinary treatment team meetings would be highly respected, and you would thrive as a vital resource of information about community resources. Additionally, you would perform the invaluable role of active ambassador to organizations that provide supportive services to our clients and their families, as well as to the general public and governmental regulatory agencies.
