Job Description Title: Licensed Vocational Nurse (LVN) Reports to: Nurse Manager Department: Residential Location: Reseda, CA 91335 FLSA Status: Non-Exempt Program Hours: 24 hour, licensed ICF/DDN residential facility. Work schedule varies depending on residents’ needs; includes weekend hours. Supervisory Responsibility: None Pay Rate: $32 – $35 per hour Position Summary Under the supervision of the Nurse Manager, provide care and supervision to the residents in the Intermediate Care Facility/Developmental Disabled Nursing (ICF/DDN) facility Essential Duties and Responsibilities Implement written plans as part of each residents’ Individual Service Plan (ISP) to provide nursing services consistent with the diagnostic, therapeutic and medication regimens as outlined by the Nurse Manager. Assist with residents’ health services and assessments as needed and directed by the Nurse Manager. Notify the Qualified Intellectual Disability Professional (QIDP) and Nurse Manager of marked or sudden change in signs, symptoms, resident behavior, etc., and of any medications errors. Document contacts and observations in residents’ charts weekly or more frequently as needed. Act as shift leader ensuring all duties are performed. Address performance issues as necessary and notify Nurse Manager using staff log. Observe and document the residential assistants’ proficiency in handling, administering and recording medications, and report to the Nurse Manager and House Manager. Participate as a member of the interdisciplinary team as needed for the following: preadmissions, resident evaluations, placement, periodic reevaluations of the type, extent and quality of nursing services and discharge planning as directed by the Nurse Manager. Participate in the Attendant Training Plan and assist with the medication training program for the residential assistants. The medication training program will cover all areas outlined in Title 22. Using ECF’s vehicles transport residents to medical appointments as needed. Other Duties Comply with ECF’s Injury and Illness Prevention Program. Ensure safe use and operation of tools and/or equipment. Comply with Bloodborne pathogens engineering and work practice controls that reduce the likelihood of exposure (e.g. appropriately handling and disposing of sharps, and proper use of personnel protective equipment). Comply with ECF’s policies and procedures including, mandated reporting requirements. Regular, reliable attendance. Perform other duties as assigned. Qualifications Education/Licenses/Certifications : Registered in California as a License Vocational Nurse. Experience: Two years working as a registered, Licensed Vocational Nurse for a Long Term Care Facility. Knowledge/Abilities/Skills: Good oral and written communication skills. Ability to work on multiple assignments with competing deadlines. Ability to perform any aspect of nursing services needed by the residents as part of the services offered in an ICF/ DDN setting. Ability to follow strict guidelines regarding sterilization of medical instruments. Knowledge of common safety hazards and precautions sufficient to establish a safe work environment. Ability to interact with all levels of the organization. Ability to deal appropriately with sensitive and confidential information. Computer literacy and proficiency in software applications including MS Office products: Word and Outlook. Requirements Valid California driver’s license, reliable transportation, evidence of car insurance, and insurable driving record. Pass a DOJ background investigation. Provide evidence of a negative TB test initially and annually thereafter. Pass a pre-employment physical and annually thereafter. Obtain and maintain NCI, First Aid and CPR certification within 30 days of employment. Work Environment ICF/DDN residential facility for adults with varying degrees of developmental and physical disabilities. Employee may encounter residents who are in crisis, exhibiting emotional/psychological distress or behaving in an aggressive manner. Environment may include frequent interruptions, loud noise, varying indoor temperature, and drafts. Exposure to varying weather conditions when driving to meetings, appointments, etc. May be exposed to bloodborne pathogens.
