Family Health Advocate
– BIH PROGRAM

Overall Function:

Under the direct supervision of the Program Supervisor, the Family Health Advocate will provide comprehensive case management services to BIH program clients.

Reports to: Program Supervisor

Essential Job Responsibilities:

1.

Conducts a comprehensive assessment of the client’s needs

2.

Develops a service plan in conjunction with the clients based on assessment

3.

Ensure client’s complete the Character Strengths Survey

4.

Assist clients to create goals and develop their Life Plan

5.

Initiates follow
– up assessments

6.

Maintains consistent contact with clients through homes visits, office appointments and telephone conversations to follow through with services and ongoing support

7.

Makes appropriate referrals and act as a resources/advocate to assist in obtaining services including accompanying clients to services.

8.

Conduct home visits within two weeks of infant’s birth and when the infant is eight months old to observe the infant sleeping area and educate clients about SIDS and Safe Sleep.

The FHA will document observations and feedback provided to mothers/parents/caregivers using the appropriate program forms.

9.

Utilize the BIH Case Management FHA Self-Assessment Tool for one (1) work week each quarter to evaluate case management skills

10.

Coordinate Case management services with Group Facilitators to reinforce weekly group session

11.

Complete ETO forms and documentation for clients and programs services

12.

Attends provider and staff meetings

13.

Networks with other involved service providers in the community

14.

Assists with other duties as assigned

Qualifications:

1.

BA Degree or enrollment in a college/university in women/maternal, child/infant health, social work, health education, or human services development

2.

Three years of experience providing maternal health services to African American women.

3.

Knowledge of socio-cultural experiences compatible for pregnant African American women and children

4.

Excellent written/oral communication skills

5.

Excellent interpersonal skills, critical thinking and problem-solving skills.

6.

Computer literate

7.

Experienced working with low income, minority and inner-city populations

Criminal Record (fingerprints) clearance will also be required.

Valid California Drivers License

5.

Have own transportation and a clean DMV record

6.

Valid/Current Automobile Insurance

Pay: $41,000.00
– $46,000.00 per year
