PGY1/ PGY2 Residency/ Acute Care Pharmacist

Rx relief is seeking a PGY1/ PGY2 Residency/ Acute Care Pharmacist to work in a hospital setting in the Los Angeles, CA area in a NON-Classified COVID-19 Facility. We are looking to fill this position quickly!

Pay/Compensation and Benefits for:

PGY1/ PGY2 Residency/ Acute Care Pharmacist

$55.00- $65.00 Depending on Experience

PGY1/ PGY2 Residency/ Acute Care Pharmacist Job Specifics and Requirements:

· Must be able to work in the Los Angeles, CA area

· Must have valid, open and active CA Pharmacist license.

· Must have Pharm D

· 1+ years of Pharmacist experience, Clinical Experience is a plus

· Background and Drug Screening

· Health Screening will be required

· Covid 19 Vaccine required

PGY1/ PGY2 Residency/ Acute Care Pharmacist Work Schedule:

· Time – Schedule Varies – Day and Afternoon shifts available

· Monday-Friday/ Weekends

· Temporary to Permanent

Job Requirements:

PGY1/ PGY2 Residency/ Acute Care Pharmacist Responsibilities:

· Review, process and monitor physician orders and patient medication profiles.

· Promote the optimal use of medications (including prevention of improper or uncontrolled use of medications).

· Educate and consult with healthcare professionals, the patients and the public in pharmacotherapy.

· Interview and evaluate patients upon admission to the hospital.

· Design a pharmaceutical care plan as appropriate and document the plan in the patient’s electronic medical record.

· Process and/or ensure the processing of all discharge medications for the assigned patient care area.

· Provide consultative services to all professional staff regarding drug information and therapy.

· Review, interpret and evaluate medication orders to identify, prevent and resolve potential or actual drug related problems.

· Documents interventions and their outcomes for use in drug therapy quality improvement efforts.

· Prepare and dispense medications using appropriate techniques and applicable policies and procedures

· Research and organize patient-specific information, and document patient medication histories.

· Recommend or review laboratory tests, and identify medication therapy problems as appropriate.

· Participate in monitoring and improving relevant outcome measures and/or quality improvement indicators

· Document pharmaceutical care activities in patients’ medical records or clinical information system.
