Administrative.
Assists DON in the records documentation process to ensure that the patient’s/client’s clinical record meets legal and regulatory requirements, facilitates care, enhances the continuity of care, helps coordinate treatment and evaluation of the patient/client, and establishes medical necessity.
Patient Care.
Provides medical oversight for clients in their homes
– including but not limited to assessments, obtaining MD orders, monthly contacts and maintaining an appropriate Plan of Care.
Responsible for coordination, and supervision of the agency’s paraprofessional staff.
Evaluation & Documentation.
Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff Assists DON in the records documentation process to ensure that the patient’s/client’s clinical record meets legal and regulatory requirements, facilitates care, enhances the continuity of care, helps coordinate treatment and evaluation of the patient/client, and establishes medical necessity.
HHA Educator.
Assists with staff development needs of employees and facilitate the educational programs to meet those needs.
Facilitates training using activity-based curriculum intended to develop the professional skills, Position Requirements
• Registered Nurse (RN) licensed to practice in New York State
• Baccalaureate Degree preferred; Associates Degree required.
• Knowledge of New York State DOH Regulations and Joint Commission Standards
• HHA Educator experience, a plus
• UAS experience, a plus
• Strong oral and written communication skills.
• Must be willing and able to travel to other locations as needed.
