For you this means stability and opportunity for advancement.Come and join an Award winning team of home health and hospice professionals!What you will do:Have a unique opportunity to help to grow our business and impact the bottom line!Responsible for identifying appropriate accounts/patients through review and consultation with health care professionals to assist in securing appropriate services and follow-up supportHelps to achieve and maintain continuity of patient care by planning and exchanging information with other professionals within the hospital, long term care facilities, physician offices, medical groups, case management offices and others in the community.Develops and maintains relationships with, and identifies needs of community organizations, utilization review nurses, discharge planners, case managers, physicians, hospitals, nursing facilities, medical groups, and other health care professionals who have an opportunity to utilize Agency servicesMaintains a level of knowledge of Agency services to present to community health care providers and payor sources.Attends and participates in rehabilitation, discharge planning and patient care meetings as requested.Provides and/or coordinates agency sponsored educational opportunities and presentations.Maintains professional growth and development through networking, membership in professional organizations and continuing education.Additional list of duties and responsibilities are provided in the job description.Who you areHigh School Diploma is required; Associate’s Degree or higher preferredAt least 2 years of client service and sale experience, health care industry experience is preferredProven ability to achieve sales goalsMust possess excellent organizational skills and be detail orientedMust demonstrate strong written and verbal communication skills and be able to interact effectively with health care professionals, patients, peers and managementAssisted Offers Our Employees a Unique Employment Package:Working in the environment where you are supported by a team of skilled healthcare professionals who are committed to providing the highest levels of care where the patient comes firstOpportunity to promote innovative healthcare services and specialty programsOur field employees enjoy flexible work hours and opportunity to work close to homeWe offer a very competitive pay.
