We’re looking for an experienced Director of Nursing to join our team The Director of Nursing is an RN responsible for the overall clinical operation of the Hospice agency.

This role oversees the dynamic interdisciplinary team and works closely with the Administrator to ensure compliance and excellent patient care in a manner consistent with the guidelines and policies of Aasta Hospice.

Your team consists of RNs, LVNs, Social Workers, Chaplains, and Home Health Aides.

Responsibilities: Coordinate and supervise the delivery of hospice services, activities and patient care provided by interdisciplinary staff/team in the home setting; formulates and monitorsquality and performance criteria, policies, procedures, and service standards.

Implement and maintain patient care and service standards in conjunction with administration.

Monitor and facilitate workflow, maximizing utilization of resources to provide the highest quality of care.

Manage staff, provide clinicals supervision and implement, deliver and evaluate/improve assigned hospice services.

Ensure compliance in meeting all regulations and standards related to hospice, Medicare, JCAHO, Title 22 and other federal, state, and local requirements.

Ensure accuracy and completeness of patient charts and documentation.

Identify service delivery/quality issues and implement corrective action/plans for improvement.

Determine the appropriate staffing requirements and develop processes to interview, hire, train and maintain the competency of all department staff.

Assess professional development needs of staff and plans for training/continuing education of professional staff.

Provide clinical leadership and support to local Aasta Hospice team.

Act as patient advocate resolving patient care issues.

Interpret and explain the program benefits to members and caregivers.

May provide liaison, monitoring, consultative expertise, orientation and continuing education with referral relationships.

Participate in special projects; attend and/or conduct meetings, committees with the objective of increasing agency market share and improving quality of patient care.

Conduct a quality management/improvement/assurance program to ensure compliance with standards for hospice services/programs and external regulatory agency requirements.

Evaluate clinical outcomes and performance of program/services and make appropriate recommendations for new modalities/programs or modification/improvement of existing programs.

Ensure practices, services and operation procedures comply with administrative, legal, and regulatory requirements of the Health Plan, governmental and accrediting agencies.

Basic Qualifications: Currently licensed as a Registered Nurse (RN) in good standing with the California State Board of Nursing.

Minimum of two (2) years of supervisory/management experience required in a licensed Medicare certified hospice or home health agency, within the last five (5) years.

Additional Requirements: Ability to demonstrate knowledge of and ability to apply state, federal, and JCAHO standards and requirements regarding hospice operations, services, and programs.

Demonstrated ability to utilize written and verbal communication skills, as well as management skills such as problem solving, budget/program planning, performance improvement, organizational planning, implementing and monitoring performance standards.

Must pass a background check/vetting procedure.
