Reporting to the Manager of Social Services, the Lead BHS is responsible for providing: mental health screenings, comprehensive mental health assessments, and psychotherapeutic services to MLKCH patients identified by the interdisciplinary team as needing specialized clinical social work intervention. The Lead BHS is responsible for the oversight and direction of the Integrated Behavioral Health program in conjunction with Social Work Management. The role reflects appropriate knowledge of clinical social work and psychotherapy as defined by the CA Board of Behavioral Sciences, and is also responsible for meeting any and all other state requirements, CMS Conditions of Participation, EMTALA, The Patient Bill of Rights, AB1203 and other Federal or State regulatory agency requirements. The BHS partners with the medical staff, and utilizes evidence based interventions in caring for MLKCH patients. This position will be supporting the inpatient team to ensure high quality care, and assist in transitioning patients to an appropriate level of care once discharged (Intensive Outpatient Programs, Out Patient Programs, and Inpatient Rehabilitation). This position requires the full understanding and active participation in fulfilling the Mission of Martin Luther King, Jr. Community Hospital. The Watson Model of Care is utilized throughout the delivery system of care services.ESSENTIAL DUTIES AND RESPONSIBILITIES* Is responsible for screening, assessing, and providing clinical social work and psychotherapy services in the inpatient setting.* Is responsible for providing resources to patients and their families in order to support recovery efforts.* Is part of the interdisciplinary team, responsible for rounding on patients in order to facilitate timely identification of mental health and/or substance abuse concerns.* Meeting facilitation of weekly IBH team meetings.* Provides ongoing education and resources for the homeless community to all Care Management staff on new programs/facilities serving the homeless community.* Participates as a successful team associate, specifically supporting data collection, health outcomes reporting, clinical audits, and pragmatic evaluation.* Facilitates networking with outpatient IBH team members.* Actively seeks out opportunities for participation in hospital committees.* Guides the development and growth of MLKCH IBH Program in conjunction with the Social Work Manager.* Develops best practices for IBH team.* Collaborates with hospital leadership and Information Technology team to develop documentation and work flow for IBH team.* Pursues continued education and best practices to share with IBH team.* Seeks to expand role to connect high utilizer patients with community resources.* Provides crises intervention to patients and/or families with challenging psychosocial issues.* Is responsible for developing a plan of care that meets patient’s needs once discharged from the acute care setting (IOP, OP, or IP rehabilitation).* Is responsible for the timely reporting of suspected abuse (child, dependent or older adult) to the appropriate agency if identified in the course of treating the patient.* Prioritizes cases and responds to triage system appropriately.* Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services in the EMR.* Timely handoff to social work colleagues in the event that the patient requires routine versus specialized clinical social work services.* Provides consultation to hospital staff and other team members about behavioral services.* Adheres to departmental and hospital wide policies and procedures.* Participates in the Quality and Performance Improvement Plan for the hospital, as it relates to the provision of clinical social work and psychotherapy services.* Considers the patient population served, age-specific criteria and the Watson Model of Care in all patient/family care and interaction.* Other duties as assigned.POSITION REQUIREMENTSA. Education* Master’s degree in Social Work* LCSW preferredB. Qualifications/Experience* Clinical Social work/Psychotherapy Experience Required (minimum 2 years) in an inpatient or outpatient behavioral health or substance abuse setting preferred* One year experience in an Integrated Behavioral Health role.* Knowledge and understanding of evidence based clinical social work interventions (CBT, Trauma Focused CBT, specialized training in substance abuse/recovery methods).* Knowledge of the classification of mental disorders contained in the current DSM V.* Ability to educate patients about signs, symptoms, and treatment regarding mental illness/substance abuse.* Knowledge of the relationship between behavioral health and physical/chronic health conditions.* Able to navigate the acute care setting and successfully connect with outside provider networks that will facilitate transitions of care for patients post discharge.C.
