QUALIFICATIONS/REQUIREMENTS:
1. Licensure/Certification:
a. Current, valid, Registered Nurse licensure to practice in the State of California
b. Current BLS certification
Experience:
a. Requires at least six months prior RN experience in the past two years in a general acute care hospital setting, or acquired equivalent competency appropriate to the type of sub- acute residents that the facility provides care to.
SPECIFIC POSITION DUTIES:
1. Check daily staffing and assure staffing needs are addressed- updating schedule for call- off and/ or additions as needed. Assures daily staffing record is correct with census and staff numbers within 1 hour of start of shift.
2. Supervises and administers direct nursing care when more than routine care is indicated. Evaluates all residents with COC to assure residents’ needs are being met. Provide support/ education to staff, residents and families.
3. Makes rounds twice each shift to assure proper care and a safe, clean resident environment throughout the facility.
4. Meets with all new admissions to assure core needs are being met.
5. Assures accuracy and completeness of medical records. Reviews resident record for all new admissions to assure complete per facility policies and procedures. Review all event reports for completion.
6. Assists with admissions, resident care plans and resident assessments and responsible for all IV needs of residents. Assists with Pharmacy and Dietary recommendations. To assure F/U with 72 hrs. Assures meal trays are delivered in timely manner
POSITION RESPONSIBILITIES/DUTIES:
1. Performs assigned resident care duties in a manner that provides for the physical, psycho-social, and spiritual needs of the chronically ill and technologically dependent resident, incorporating cultural and ethnic factors.
2. Completes initial and ongoing assessments in a timely manner, incorporating functional/development age factors into the assessment process.
3. Correctly differentiates between normal and abnormal clinical findings and intervenes in accordance with clinical standards of practice and per physician orders.
4. Works collaboratively with the resident/family and interdisciplinary team members to develop an individualized plan of care for each resident.
5. Demonstrates sound clinical judgment in the implementation, and evaluation of the nursing aspects of interdisciplinary resident care plan of care.
6. Proficiently and accurately evaluates, assists with treatments, and reports resident condition changes to the attending physician, family, interdisciplinary team members and nursing personnel.
7. Directs and coordinates the care of each resident to meet their overall individualized needs.
8. Administers medications in a proficient manner, including IV therapy and pain management. Is able to accurately calculate dosage and solutions appropriate to resident’s age, as needed, including IV therapy, IV pain management, and TPN/PPN. Is able to provide care and treatment to residents with Central Lines, PICC lines, and peripheral IV’s.
9. Provides treatment administration in a proficient manner per direction from the physician, which includes but is not limited to: wound packing, irrigation, indwelling catheter care, etc.
10. Proficiently provides and manages the care of technologically dependent residents, including but not limited to:
a. Ventilator dependency
b. Ostomies such as, but not limited to: tracheostomy, ileostomy, urostomy, gastrostomy etc.
c. Enteral feeding tubes
d. Tracheostomy Care
e. Suctioning
11. Proficiently provides care in emergency situations and follows established emergency procedures and practices.
12. Maintains knowledge of and demonstrates the ability to identify therapeutic resident care issues/problems and determines measures and interventions to promote, maintain, and/or restore health of residents.
13. Assures that the rights of residents are respected and maintained by allowing for privacy and dignity in the provision of care.
14. Identifies resident/family educational needs; initiates and provides individualized teaching which considers the functional/developmental age and needs of the resident. Evaluates and documents the effectiveness of the education provided.
15. Demonstrates knowledge of age specific care, including but not limited to identifying safety measures, physiological normal values/reading; and assessing skin integrity, behavior, motor skills and/or activities that place the residents at risk as well as communicates effectively in a clear, concise, and understandable manner.
16. Demonstrates knowledge and appropriate assessment of signs/symptoms of abuse and/or neglect of residents. Reports and documents suspected abuse/neglect in accordance with facility policy and regulatory guidelines.
17. Demonstrates good oral and written communication skills with residents, families, physicians, additional healthcare providers, and co-workers.
18. Timely, accurately, and thoroughly prepares documentation in a manner that conforms to prescribed style and format, and to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
