HIV Testing Counselor

OverviewThe PrEP Navigator provides quality and comprehensive HIV/STI Health Education, HIV counseling and testing, and facilitates HIV Prevention and PrEP/PEP Navigation Services to clients at high risk for infection or transmission of HIV. Core responsibilities include: 1) referring clients for ACA enrollment, 2) navigating clients onto PrEP/PEP services, 3) scheduling medical appointment for PrEP/PEP, and 4) assisting with Medication Assistance Programs. Conducts program recruitment for PrEP, client intake and assessment, facilitation/mentoring, HIV counseling and testing, as well as coordination of referral services into other programs as appropriate and necessary. The PrEP Navigator monitors, evaluates, and reports on progress toward grant deliverable. The PrEP Navigator is also expected to represent AltaMed by serving on internal and external workgroups, collaborative, and coalitions. All program activities will address and empower each client’s efforts to reduce the risk of HIV infection or transmission and assist in community building in AltaMed’s service area.ResponsibilitiesProvide HIV/STI testing on the Mobile unit, in ‘storefront’/clinical settings, and in field-based locations. Conduct risk behavior screenings to provide basic PrEP/PEP information and other HIV prevention options as necessary. Coordinate program intakes and assessments with PrEP/PEP clients. Conduct referrals as needed based on the assessments conducted at program intake and follow-ups. Manage and track the referral of clients to medical care and necessary support services in the community. Provide education about benefits that a client may be eligible for and/or explain how best to use existing benefits. Coordinate patient appointments for PrEP/PEP initial and follow-up medical and non-medical visits Provide PrEP/PEP education regarding adherence, missed dose protocols, side effects, and symptoms of seroconversion Coordinate patient appointments for PrEP/PEP initial and follow-up medical visits Accurately complete required documentation for HIV Prevention programs including HIV counseling and testing forms, client satisfaction surveys, incentive logs, program forms, and others as determined by HIV Prevention Team leadership. Participate in regular internal audits to ensure program documentation, charts, and reports are accurate and completed monthly. Promote HIV Prevention Team activities through in-service presentations to AltaMed staff and other partnering agencies. Participates in program, departmental, division, and organizational staff meetings. Completes comprehensive training for HIV Prevention Program staff including, HIV test counselor training, group facilitation, client-centered counseling, harm reduction, etc. as required by HIV Prevention Team leadership, grant requirement and/or local health authority. Other duties as assigned and relevant to core activities of the HIV Prevention Program.QualificationsHigh School Diploma or equivalent required. Bachelor’s Degree in social services or other health-related field preferred. A minimum of one (1) years of experience in a human services field required (e.g. mental health, substance use disorder services, homeless services, etc.), previous experience providing HIV prevention or case management services strongly preferred. A minimum of one year professional experience in Motivational Interviewing and/or demonstrated capacity to deliver client-centered harm reduction counseling highly desirable. Previous experience delivering case management services to low income, at-risk Hispanic populations preferred. Existing certification as a HIV Test Counselor Los Angeles Division of HIV and STD Programs strongly preferred; or must be willing to complete HIV Test Counselor training within 3 months of employment with AltaMed.
