Job Purpose or Summary:
In alignment with company mission, vision and values, we are all about creating better care experiences for our older clients and/or their involved family member:
On-Demand Caregiver Support: The Life Care Manager provides support and guidance to involved family caregivers through phone, text, video call venues helping the involved family member better manage the needs of their older loved one.
Traditional Care Management: Coordinates and manages service delivery to clients, optimizing their ability to remain independent and safe at home.
Care Managed Home Care Providing support, education and oversight to caregiver employees to ensure they have what they need to provide quality care for our older clients.
Essential Duties & Responsibilities:
- Uses professional care management skills to independently assess and re-assess client status, including physical and mental health, social resources, environmental safety, economic resources, and functional ability.
- Drafts, implements and revises the client Plan of Care and Caregiving Plan, to ensure appropriateness, effectiveness, efficiency, and client satisfaction.
· Collaborates with families, guarantors and other service providers to facilitate care coordination and information sharing within HIPAA and other regulatory guidelines.
· Ensures effective communication with clients, families and in-home staff through ongoing home visits, meetings with members of the client committee, telephone contact, email updates, and written documentation.
· Provides supportive counseling to and conflict resolution with clients and family members; serves as an advocate for the well-being of the client.
· Makes referrals to appropriate community agencies such as crisis intervention, legal advocacy, and counseling agencies for medical, mental, physical, financial, legal, and social needs.
· Communicates elevated care issues with appropriate internal professionals, including Chief Professional Officer, Senior Vice President of Operations, etc.
· Extends client expected length of stay (ELOS) through the delivery of exceptional care management services and consistently exceeding client expectations.
· Interviews, matches, trains, and supervises in-home staff to ensure client needs are met.
· Maintains productivity targets based on company guidelines.
· Anticipates client/caregiver issues in order to maintain quality care within client home, and investigate potential risk management issues.
· Performs all responsibilities in accordance with professional codes of ethics, JFS Care policies and procedures, and applicable state and federal regulations.
· Serves as Care Manager on call, receiving pages and telephone inquiries evenings and weekends during on call rotation.
· Documents and updates activity in multiple systems to ensure compliance with company protocols.
· Assists with education curriculum development and presentations.
· Collaborates with community organizations to promote awareness and understanding of elder care issues.
· Other duties as assigned.
Specifically for the On-Demand Caregiver Support
- Outreach / Sales and Marketing of program – educational presentations, speaking to groups, working with Marketing to develop relevant tools and collateral material.
- Comfort with technology, and collaboration with technology partners to ensure that “family application” is working to our required specifications.
- Program development skills required to create work-flows, tools and documents to support program
- Strong interpersonal and clinical skills to engage with family members offering value to help them along their eldercare journey
Supervisory Responsibilities
· Supervises in-home staff, including safety monitoring, performance management, training and all other supervision.
