Job Description Responsibilities of an RN Case Manager Include: Manage 10-20 patients. Maintain and document patient records. Monitor if changes are made with medication or new doctor’s order. Documentation per agency policy that is consistent with patient plan of care. Universal precaution of patient’s environment Assessments from head to toe. Ensuring all clinical administration is done with accuracy . Training / Orientation: Training is provided in all clinical administration. Care Type: To care for pediatric and adult patients dependent on G-Tube , Tracheostomy and Ventilator.
