Housing Case Manager
Job Summary
The Alcott Center for Mental Health is seeking an organized, motivated and professional candidate for a full-time position providing housing case management services in its Outpatient Program.
The population served includes adults with a full spectrum of DSM diagnoses.
The individual will provide treatment planning, linkage, crisis, community outreach and rehabilitation services as part of a multi-disciplinary team.
Responsibilities:
· Able to act as a liaison between the agency and housing placements
· Able to search, arrange and coordinate client housing placements
· Able to coordinate medical follow-ups for clients with concurrent conditions
· Able to provide life skills training
· Able to document provided services in an accurate, thorough, and timely manner according to LAC-DMH and Medi-Cal documentation standards
· Able to follow detailed instructions and independently complete assigned tasks
· Must demonstrate effective interpersonal, oral, and written communication skills
· Ability to collaborate effectively in a team setting
· Ability to perform basic computer skills required for an electronic health record
· Able to consistently maintain agency productivity standards
· Other duties may be assigned as needed
Qualifications:
· BA/BS in psychology, social work, counseling, or related field
· Experience with LA County Department of Mental Health paperwork preferred
· Evidence of clear criminal record, eligibility to claim Medi-Cal, and proof of valid California Drivers License
· Bilingual in Spanish-English preferred, but not required
Job Type: Full-time
Pay: $22.05 per hour
Schedule:
8 hour shift Monday to Friday
