LVN Case Manager

Job Description Position Purpose: The LVN Case Manager provides comprehensive case management services to chronic disease management program patients. Performs vocational nursing duties in the Wesley Health Centers. Participates as a member of a multidisciplinary team to ensure patients are linked to a full range of medical and support services to ensure maintenance of health and well-being. Principal Responsibilities: Conduct comprehensive initial and ongoing client assessments to include the functional, psychosocial, housing, mental health, substance abuse and educational status. Work with chronic disease management program clients to develop individualized service treatment plans (ISP) with measurable goals pertaining to housing, mental health, substance abuse, education, life skills and other challenges applicable to the population, as indicated. Monitor the care plan (ISP) to ensure that there is progress toward goal achievement. Maintain case management progress notes on all clients being followed to document progress Act as patient advocate and identify resources and provide referrals that will help the client meet the goals of the ISP, including housing, mental health, substance abuse and vocational services. Monitor activities to ensure that there is client follow-through and that services are being delivered Participate in weekly multi-disciplinary meetings with the medical and nursing personnel to coordinate the care and discharge (placement) of clients Assist in the preparation of written and/or electronic (HMIS) reports required for continued funding Maintain accurate client records and patient confidentiality. Build and maintain positive working relationships with co-workers, other social service agency personnel, and the clients. Provider vocational nursing duties in the clinic. Perform related duties and responsibilities as assigned. Requirements: Current and valid California LVN License. Current BLS card from the American Heart Association. Possession of a Bachelor’s degree from an accredited college or university with major course work in social work, sociology, psychology counseling or a related field and one year of professional case management experience OR any combination of experience and training that would provide the required knowledge and abilities to perform the job responsibilities listed above. Ability to plan, develop, implement and monitor progress toward completion of case management (ISP) plans. At least one-year experience in an ambulatory care setting. Ability to communicate effectively orally and in writing. Experience in working with clients and staff from diverse socio-economic, ethnic and cultural backgrounds. Current and valid California Driver’s License with current car insurance. All JWCH, Wesley Health Centers workforce members must be fully vaccinated against COVID-19 as a condition of employment.
