This organization offers a competitive salary, full benefits, and much more Job Summary: Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment and nursing home care. Attends scheduled IDT and Coordination meetings. Skills and Abilities: 1.Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred. 2.Ability to work well with patients. Requires lots of patience, and a kind/caring attitude. 3.Must be able to work effectively in a team environment 4.Only act within the scope of his or her authority to practice. 5.Must be able to treat geriatrics. 6.Demonstrates respect for and promotes participant rights including dignity, self-determination, access to care, confidentiality, and independence. Physical Requirements : 1.Walking, talking, standing, sitting, bending, stooping, and lifting up to 10 ‘s. 2.Talking and hearing are essential to effectively communicate with patients, clients, and staff. 3.Utilize correct body mechanics to transfer, lift and position participants Education and Experience: 1.Graduation from an accredited LVN school coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required. 2.A minimum of one (1) year experience practicing as a Licensed Vocational Nurse. 3.Current CPR/BLS certification also required. 4.Experience and knowledge regarding the physical, mental and social needs of the patients highly desirable .5. At least one year working with a frail or elderly population6.Valid California driver’s license with good driving record. Job Functions: 1.Assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops specific plans of care. 2.Assess, identify and collaborate with other members of the Interdisciplinary teams regarding all Durable Medical Equipment and Incontinence care needs of the participant. 3.Perform any follow-up as instructed by the Provider. 4.Collaborate with the Interdisciplinary team to evaluate and re-evaluate caregiver hours. 5.Discuss and educate approved caregiver hours and assignment plan with participant, family and caregiver .6. Actively participates in Interdisciplinary Team (IDT) contributing to the participant plan of care. Communicates any changes in participant. 7.Attends staff, scheduled IDT and Care Coordination meetings. 8.Document in accordance with the company’s departmental guidelines and within the established time frames 9.Act as a liaison between Homecare vendor and PACE related to participant’s needs. 10.Maintains patient confidence and by keeping information confidential .11. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. 12.
