A Place Called Home is a dynamic, non-profit youth and community center in South Central Los Angeles.
APCH provides educational programs, counseling, mentoring, music, dance and art classes, fitness and health and wellness opportunities.
As youth participate in our programs, they discover interests and talents and develop a better sense of themselves and their place in the world around them.
Our goal is to increase the likelihood that youth will stay in school, pursue higher education, attain viable jobs, and become active community members capable of making positive change in the world in which they live.
Our mission is to serve youth and families directly and the optimal way to do that is in person.
The majority of our positions require employees to work onsite at our facilities.

Job Title: IMPACT BEHAVIORAL SPECIALIST / CASE MANAGER
Department: Teen & Young Adult Services
Reports to: IMPACT Program Manager
FLSA Status/Hours: Non-Exempt, 40 hours/week (Must work Saturdays)
Summary
Under the direction of the IMPACT Program Manager, the IMPACT Case Manager provides oversight, resource support and behavioral management support for our IMPACT members and families.
The IMPACT Case Manager monitors the members experience, advocating on behalf of the members human services needs.
The IMPACT team works very closely with our RISE School Partnership that is housed on APCHs site.
As a part of the IMPACT team the IMPACT Case Manager will be responsible to assist with addressing escalating situations to respond and care for members.
The IMPACT Case Manager is responsible for leadership in developing individual program plans in response to members behavioral needs.
The IMPACT Case Manager will identify, prevent or de-escalate injurious and destructive behavior in the classroom and on campus and will provide behavior intervention and socio-emotional support in the most severe behavior circumstances.
The IMPACT Case Manager is responsible for creating behavior modification plans and tools for various members across the campus site, particularly focusing on the Opportunity Youth population (RISE High and IMPACT programming).
The IMPACT Case Manager will guide the members in creating and completing SMART goals, identifying both short and long term goals.
The IMPACT Case Manager assists in the design and implementation of behavior support plans; and works with the Counseling team and all staff to implement and monitor the progress of intervention plans in accordance with A Place Called Homes policies and REACH values.
This position will coordinate with the RISE partners and the Counseling team to support, advocate and connect APCHs youth and their family members to resources according to need.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Case Management
Under the direction of the Program Manager and in collaboration with Membership and Counseling departments, facilitates the member intake process, which includes a needs assessment, SMART goals and complete a Member Intervention Plan (MIP) that outlines the program plan that aligns with their goals.
Conduct 1:1 meetings on a monthly basis with members to evaluate progress towards goals, identify challenges and solutions, and to adjust the program plan as needed.
In collaboration with Ed Services, Higher Education, and Professional Readiness and Employment staff, monitor progress members are making on educational and/or transitional plans, MIPs and goal setting sheets and update Program Manager on a consistent and as needed basis
Manages case load (case management) between 25-50 members
Responsible for the accurate and timely record keeping
Must maintain an accurate case management tracking database in which each profile includes an Inquiry Form, SMART Goal Setting Sheet, Needs Assessment, and Member Intervention Plan and other documents as required; update case files within 48 hours of meeting with members
Responsible for administering the youth assessment tools (e.g.
Adverse Childhood Experience (ACE) Questionnaire or the Beck Depression Inventory), and other onboarding documents as required
Works closely with Program Coordinator to provide member referrals and support with the dissemination of resources when necessary
Maintain confidentiality requirements at all times
Provide referrals to resources and service both internally and externally
Performs related duties as assigned
Coordinate Community Resources
Assists with researching and connecting with potential partner organizations for purposes of resources, special opportunities, and to build and maintain a referral base for direct connect.
Research and be involved in connecting IMPACT youth to community resource events
Support holistic care for members by connecting each member with appropriate APCH departments and resources
Assist with identifying needs of the program and members
Responsible for providing referrals to resources both internally and externally
Support expense tracking for all resources purchased by IMPACT program to ensure accurate tracking of grant expenditures
Coordinate and disburse internal resources to members
Collaborate with the In-Kind team and Counseling department to maintain an accurate inventory of resources available to members on-campus
Program Coordination
Collaborate with the Program Manager and Program Coordinator to set the calendar of events, special opportunities and program offerings for each semester
Assist Membership with placing member in the appropriate classes each semester as outline in individuals Member Intervention Plan
