Hiv Case Manager

JOB SUMMARY: Provide services for clients identified with HIV/AIDS.

Utilize community resources to assist patient in understanding and coping with HIV/AIDS.

Participate in planning for improved health outcomes by ensuring adherence to treatment plan.

Work in collaboration with physicians, nurses and other staff in issues pertinent to client treatment and adherence.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES: 1.

Provide medical case management under the Ryan White Grant to HIV infected individuals who live in Houston Eligible Metropolitan Area (EMA) and Houston Service Delivery Areas (HSDA).

2.

Build and maintain information and referral services to provide to all identified patients.

3.

Provide health education and information to HIV infected clients.

4.

Maintain data with respects to demographics, services rendered, and barriers to care.5.

Liaison between AccessHealth and other social services agencies.

6.

Submit statistical information on clients served to Director of Behavioral Health & HIV Services and other sources as required.

7.

Maintain accurate and up-to-date case records and files.

8.

Enter all required data into CPCDMS (Centralized Patient Care Data Management System) as required for Ryan White grant management and billing.

9.

Complete all Ryan White required processes to determine program eligibility.

10.

Complete all required client assessments, re-assessments and care plans in a timely manner.

11.

Comply with all Ryan White Standards of Care in providing client services.

12.

Assist clients in securing adjunct services, to include medication and transportation assistance.

13.

Coordinate with other AccessHealth departments to provide a cohesive program of services for HIV/AIDS clients.

14.

Attend all Ryan White meetings and training per grant requirements.

15.

Comply with all company policies, procedures and regulatory requirements.

16.

Performs all other duties as assigned by management.

EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS: REQUIRED: 1.

A Master’s degree in Social Work from an accredited college/university.

2.

Must be licensed by the Texas State Board of Social Work Examiners (LMSW) SKILLS AND EXPERIENCE: REQUIRED:1.

High ethical standards.

5.

Well-organized and self-directed.

6.

Maintains strict patient confidentiality.

7.

Ability to handle multiple projects.

8.

Ability to work with people of all socioeconomic levels and cultures.

9.

Must be a consumer advocate for quality health care and service PREFERRED: 1.

Experience in health care setting preferred.

2.

Experience working with HIV individuals preferred.3.

Bilingual English and Spanish preferred.

The noise level in the work environment is usually moderate Hours: Employees will be assigned to a regular work schedule, which may include hours before 8:00 am, after 5:00 pm, and Saturdays as determined by department needs and requirements.

May travel to other AccessHealth sites to provide services as needed.
