The Case Manager is responsible for a variety of tasks that lead to a high level of customer satisfaction in the most cost-effective manner. Responsible for case management activities(including utilization management and discharge planning activities) on designated units within the hospital. Qualifications Required Minimum Education: Licensed Vocational Nurse Degree or Associate Degree in Nursing. Bachelor’s Degree in Nursing preferred. Specialized Training: Case Management, Utilization Management or Discharge Planning preferred. CCM or ACM certified preferred. Licensure: Licensed Vocational Nurse or Registered Nurse with current and valid California License. Software: Basic Microsoft computer skills preferred.
