1736 Family Crisis Center offers crisis intervention services, including four 24
– hour hotlines, as well as longer-term services in client advocacy and outpatient psychotherapy.
The residential counseling and support services include shelter programs for (1) runaway and homeless adolescents, (2) domestic violence and human trafficking victims and their children (emergency and transitional programs (3) Homeless prevention and rapid re-housing services for the general population and Veterans and their families.
Community outreach programs are provided to educate the public and prevent mental-health problems in the general population and in targeted groups to at-risk populations.SummaryThe Case Manager works directly with domestic violence clients and their families by helping to outreach to the community and developing goal plans to meet both the clients immediate housing needs, providing crisis intervention and resource referrals as well as outreaching to homeless landlords in the community.
Maintains computerized case management and hard copy files and is responsible for special projects as assigned.Responsibilities:Assess new clients during screening, intake and orientationAssess program qualification criteria of client and aid in qualifying or referring to outside resourcesPoint person for domestic violence casesScreen domestic violence victims that enter the CES systemApply knowledge of safety planning and confidentiality for domestic violence victimsPerform training for CES staff on domestic violence victim issuesRefer domestic violence victims to wraparound services when necessary and ensure appropriate interventions including potential shelters, rapid rehousing and outpatient programs/servicesDevelop individualized goal plans and monitors progress in achieving goalsAssist veterans in goal planning of housing attainment and sustainability including job training, education, money management, and income and/or benefits attainment.Provide ongoing support, crisis intervention and housing stability support and resource referrals.Coordinate housing, medical, legal, dental and/or psychological services as/if neededWork consultatively with other staff to ensure appropriate levels of treatment/supportKeep client files current through the internal computerized care management system and maintain ongoing files both on computer and hard copiesKeep all client paperwork in a confidential mannerConduct home visits to support client in housing stabilization.Participate in conferences, workshops, special projects, staff meetings, and other duties as assignedQualifications:Bachelors Degree preferredThree (3) or more years of work experience is preferredKnowledgeable of substance abuse issues, therapeutic interventions, and treatment planning, particularly as relevant to Therapeutic CommunitiesSome knowledge of affordable housing programs such Section 8 housing, VASH voucher and Shelter Plus Care programsWorking knowledge of community resources as they relate to homelessness and domestic violenceStrong interpersonal and oral and written communication skills.
