Seeking a full-time employee as a Case Manager to work as part of multidisciplinary team for the Health Homes Program.
This program is designed to provide in-person, community-based case management and wraparound services to eligible Medi-Cal members with multiple chronic conditions.
Duties include but are not limited to: provide enhanced case management services to identify and link eligible participants to project services; Participate in outreach efforts at community sites and in-reach at TTC service site; provide follow-up as needed to engage potential participants or those who have fallen out of care.
Prior case management experience preferred as well as experience working with individuals with mental illness and substance abuse disorders.Candidate must be available to work evenings, and weekends.
Job location can be discussed with openings available in Lancaster, Woodland Hills, and Long Beach.
Bi-lingual Spanish/English preferred BA/BS/BSW degree in social work or related fields or 3 years prior case management experience To apply, please complete online application and attach cover letter and resume
